September 2024: F688 (severity G) after a resident developed a hand contracture
According to the 2567 report, inspectors found the facility did not provide range-of-motion services to three residents with limited movement. Resident 43 was admitted in August 2023 with hemiplegia (paralysis of one side of the body) on the left side after a brain bleed. Inspectors found the facility did not take baseline range-of-motion measurements of the resident's arms and legs on admission, as its own policy required, or of the left arm during the occupational therapy evaluation.
When occupational therapy ended in September 2023, the therapist recommended restorative nursing exercises for the left arm and a left wrist-hand orthosis (a brace that holds the wrist and fingers in position to prevent contractures). The report states the facility did not check the brace for fit and wear time, did not provide the restorative services from September 11 to 20, 2023 and from October 20 to November 30, 2023, and did not monitor changes in the resident's range of motion for more than six months. According to the report, Resident 43 developed limited movement in the left shoulder, elbow and hand, including a left-hand contracture (a tightening of muscles and tendons that stiffens a joint).
Inspectors found the facility also did not take baseline measurements for Resident 5 or monitor that resident's range of motion for about 10 months, and did not position Resident 18's hips at midline in bed. The report states this put both residents at risk of further limits. This was cited as F688 (severity G).
April 2024: Class A citation and F684 (severity G) after a resident's ankle surgery wound became infected with hardware exposed
According to the citation, CDPH received a complaint on February 21, 2024 and made an unannounced visit on February 29. Resident 1, a 75-year-old woman, had surgery in December 2023 to fix a broken right ankle with surgical hardware and returned to the facility in January 2024.
CDPH found that on February 7, 2024, the orthopedic surgeon ordered staff to stop using antibiotic ointment on the incision and to use Betadine-soaked gauze. Treatment nurses TN 1 and TN 2 kept applying ointment until February 10, and the records showed no Betadine gauze treatment. On February 10, TN 1 saw surgical hardware visible through the incision, and on February 20 TN 2 saw it too, but CDPH found neither nurse notified the physician or the surgeon or documented a change of condition. TN 1 told inspectors she waited for the surgeon appointment on February 21. At that visit the surgeon found drainage and an exposed screw and sent the resident to the emergency room.
According to the citation, the incision became infected, and at a hospital the resident had surgery to remove the infected hardware, clean out the wound and place a wound vacuum (a device that draws fluid from a wound). The department also found staff training and supervision were not adequate. CDPH issued a Class A citation for patient care with a $25,000 penalty, and the 2567 report cited F684 (severity G).

CDPH substantiated at least one allegation in 8 of them. 5 intakes involved an allegation in CDPH’s abuse category, and 0 of those had an abuse allegation substantiated. Cal Health Find lists 1 intake for Paramount Convalescent Hospital from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 0 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. | 2023 | 1 | 0 |
Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
