August 2023: Class A citation and F688 (severity G) after a resident developed a hand contracture
CDPH visited the facility in July 2023 to investigate a complaint of employee-to-resident abuse and did not identify a deficient practice on that allegation. According to the citation, the department instead found that Resident 1, admitted in February 2022 with no limits in arm movement, did not receive the full range of motion exercises his physician ordered. Starting in March 2022, he was to receive active range of motion exercises to both arms five times a week through the restorative nursing program.
Inspectors found the restorative nursing aides believed the order covered shoulders, elbows and forearms but not hands, while the regional physical therapy director said it included hands and fingers. The facility kept no records showing the aides were trained or competent in range of motion exercises. According to the citation, the facility did not ensure he received these exercises from March 28, 2022, through July 11, 2023, and restorative nursing summaries had no documentation of hand or finger exercises from March 28 to July 11, 2023. The resident told inspectors he could no longer straighten four fingers on his left hand, could not hold utensils with it and felt pain and frustration. An occupational therapy evaluation in July 2023 found a contracture (a tightening that keeps a joint bent) in those fingers, which had been within functional limits in 2022.
According to the citation, the failures left Resident 1 with pain when moving his left hand and unable to use it for daily tasks. CDPH issued a Class A citation and assessed a $25,000 penalty, which the facility appealed. The federal 2567 report cited F688 (range of motion and mobility) at severity G.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
