Sources, limits and corrections

Sources. Facility details come from the California Department of Public Health (CDPH) licensing file and the Centers for Medicare & Medicaid Services (CMS) Provider Data Catalog (ratings, staffing, fines, chain names). Complaints, facility-reported incidents, state enforcement actions and survey deficiencies come from CDPH’s Cal Health Find, for 2023 to the date shown on each page. State citation notices and CMS-2567 inspection reports are downloaded from Cal Health Find and linked under every finding.

What we write. For every Class AA or Class A state citation and every federal deficiency rated G or higher (actual harm or immediate jeopardy), we summarize what the government document says happened, attributing each statement to the agency. We quote the citation class and federal tag exactly as the record shows them. Residents are described only as the documents describe them, which is already anonymized. Staff are referred to by role; we name an individual only if a prosecutor has publicly charged that person.

What a citation is not. A citation or deficiency is a regulatory finding. It is not a court judgment, and facilities can appeal. Where a facility appealed, the record says so. Criminal defendants are presumed innocent, and civil lawsuit allegations are claims, not findings.

Corrections. If you believe something on a page is inaccurate, call (310) 552-6959 or use any form on the page and write “correction” with the page address. We review every correction request within five business days and update or remove anything we cannot support with a government record.

Updates. The data is refreshed regularly. A page’s “Updated” date changes only when its underlying record changes.

Important: This page reports findings from public records of the California Department of Public Health and the Centers for Medicare & Medicaid Services. A citation or deficiency is a regulatory finding by a government agency; it is not a court judgment and not proof of civil or criminal liability. Criminal defendants are presumed innocent unless proven guilty. Lawsuit allegations are claims, not findings. This page is general information, not legal advice, and reading it does not create an attorney-client relationship with Beverly Law. Attorney advertising.
