April 2026: Class A citation and F689 (severity G) after an unsupervised visit and emergency Narcan treatment for a resident

According to the citation, Resident 1 was a 67-year-old woman on a ventilator (breathing machine). CDPH found a family member, FM 2, had documented evidence of providing her with illicit substances at this facility and at her previous facility, where his visits had been put under supervision. The citation states that on February 5, 2026, after FM 2 left, her ventilator alarmed and she was breathing four times a minute.

CDPH found that on February 27, 2026, the administrator, who had restricted FM 2 to supervised visits the day before, asked another family member, FM 1, to supervise the visit. That afternoon, Resident 1's ventilator alarmed and staff found her hard to wake, with slow breathing, low oxygen and pinpoint pupils. Staff gave Narcan (naloxone, a medicine that reverses opioid overdose) and she became responsive. According to the citation, a white powdery substance was found among her belongings, and she was taken to an acute care hospital. The director of nursing told inspectors facility staff, not a family member, should have supervised the visit, and that there was no care plan addressing this risk.

The department determined the facility did not follow its own supervision policy and did not develop a care plan for the risk identified in her records. CDPH issued a Class A citation with a $25,000 penalty. The 2567 report from the March 2026 visit cites F689 (accident hazards and supervision) at severity G for the same events.

August 2024: F689 (severity G) after a resident fell from bed during a one-person mechanical lift transfer

The 2567 report states Resident 1 had one-sided paralysis after a stroke and dementia, was assessed as at risk for falls, had a history of falls with injury, and depended on staff for all transfers. An occupational therapist told inspectors he needed a mechanical lift and two people for transfers. Inspectors found his self-care care plan did not say how staff should move him between surfaces.

According to the 2567 report, at about 9:20 a.m. on July 26, 2024, CNA 1 used a mechanical lift by herself to move Resident 1 from a shower chair to bed. CNA 1 told inspectors that while she was removing the lift sling and linens from under him, he was too close to the edge of the bed, his leg slipped and he fell to the floor. She said she had nine residents that day and felt she could not ask for help. A licensed vocational nurse told inspectors she found him on the floor bleeding from his head.

Inspectors found Resident 1 had two cuts on his scalp and a skin tear on his right arm. He was taken to an acute care hospital, where the cuts were closed with stitches and he was admitted to a heart monitoring unit; he was still in the hospital on August 7, 2024. The administrator told inspectors the fall would not have happened with two-person help. The facility was cited under F689 (accident hazards and supervision) at severity G.

CDPH substantiated at least one allegation in 67 of them. 25 intakes involved an allegation in CDPH’s abuse category, and 3 of those had an abuse allegation substantiated. Sexual abuse reports at Meadow Creek Post-Acute

Cal Health Find lists 1 intake for Meadow Creek Post-Acute from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (0 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
