June 2023: Class A citation and F688 (severity G) after a resident's walking ability declined in the restorative nursing program
According to the citation, CDPH found the problem during an unannounced recertification survey that began on April 18, 2023. Patient 43, a 55-year-old man with weakness and paralysis on his left side after bleeding in the brain, had worked with physical therapy until he could walk about 200 feet with a hemi-walker (a one-sided walker) and light hands-on help. When therapy ended on November 10, 2022, the therapist recommended a restorative nursing program, in which aides help residents keep their strength and mobility, with walks of 200 feet five days a week.
CDPH found the restorative nursing aide did not follow that plan. The aide told inspectors the patient usually walked about 25 feet, never more than 50, and never 200 feet, even though records showed 200 feet completed on many days. The patient told inspectors he did not walk with the aide consistently and was scared of losing the ability to walk. Inspectors found his decline was not reported to physical therapy. By April 2023 he needed maximum help to stand and two people to walk 20 feet. The rehabilitation coordinator stated the decline was avoidable, and the director of nursing stated it was preventable and should have been reported.
CDPH issued a Class A citation with a $25,000 penalty, and the department determined the violations presented either imminent danger or a substantial probability of death or serious physical harm. The federal 2567 report from the same survey cited F688 (Increase/Prevent Decrease in ROM/Mobility) at severity G.
June 2023: Class A citation and F684 (severity G) after a resident with diabetes went months without blood sugar checks
According to the citation, CDPH found the problem during an unannounced recertification survey that began on April 18, 2023. Patient 39, a 59-year-old woman with type 2 diabetes who received her nutrition through a feeding tube and could have nothing by mouth, had no blood sugar checks documented from December 1, 2022, to April 11, 2023, and no physician order to monitor her blood sugar. Her care plan told staff to watch for and report signs of high blood sugar, such as increased thirst.
Inspectors found staff did not document her constant thirst or report it to her physician. Two licensed vocational nurses told inspectors she was always asking for water, and on the first day of the survey she asked inspectors for water or juice. On April 12, 2023, she had slurred speech and her blood sugar was too high for the meter to read. She was given insulin and sent to a hospital, where her blood sugar was over 700 mg/dL and she had a high sodium level and dehydration. The hospital admitted her to the intensive care unit and diagnosed hyperosmolar hyperglycemic state, a severe complication of diabetes caused by very high blood sugar.
The director of nursing told inspectors nurses should have reminded the physician that she had diabetes and obtained an order for monitoring, and that the hospitalization could have been prevented. CDPH issued a Class A citation with a $25,000 penalty, and the department determined the violations presented either imminent danger or a substantial probability of death or serious physical harm. The federal 2567 report from the same survey cited F684 (Quality of Care) at severity G.

CDPH substantiated at least one allegation in 29 of them. 38 intakes involved an allegation in CDPH’s abuse category, and 14 of those had an abuse allegation substantiated. An intake can contain several allegations; “unsubstantiated” means CDPH did not find enough evidence, not that nothing happened.
Sexual abuse reports at Pomona Vista Care Center
Cal Health Find lists 1 intake for Pomona Vista Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (0 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 1 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.
| Year | Intakes with a sexual abuse allegation | Substantiated |

| 2023 | 1 | 1 |
Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
