What inspectors found at Country Oaks Care Center

April 2023: F740 (severity K) after a resident repeatedly left the facility unsupervised without a physician's order
The 2567 report states the facility did not provide Resident 1 with the behavioral health care she needed. According to the report, the facility did not set goals or nursing interventions for her, even though she left the facility on passes unsupervised and without out-on-pass orders from her physician, and returned with aggressive behavior toward staff. Her February 2023 care plan noted she yelled at staff, got close to their faces and recorded them without consent, but it had no interventions for that behavior.
Inspectors found that on April 10, 2023, a transportation company dropped Resident 1 off at the facility at 11:50 a.m., but she did not check back in. She later called to say she had been in a car accident and was found in a hospital emergency room, where records show she was rear-ended and had head pain. The report states police picked her up at City Hall on April 13 while she was out on pass. Inspectors also found she went into unauthorized areas and video recorded two other residents without their consent, and the social services director reported to police that Resident 1 rammed the director twice with a walker.
According to the report, CDPH called an immediate jeopardy on April 25, 2023, finding the situation put Resident 1 and three other residents at risk of injury. The facility's removal plan included sending Resident 1 to an acute care hospital for evaluation, evaluating and monitoring the other residents, and training staff on out-on-pass orders and on assessing residents before and after they leave. CDPH lifted the immediate jeopardy on April 27, 2023. The facility was cited under F740 (behavioral health services) at severity K.

CDPH substantiated at least one allegation in 37 of them. 43 intakes involved an allegation in CDPH’s abuse category, and 11 of those had an abuse allegation substantiated. Cal Health Find lists 4 intakes for Country Oaks Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (3 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. | 2023 | 4 | 0 |
Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
