·
August 2026: Class A citation and F600 (severity G) after a resident with a history of aggression struck another resident, injuring her eye

According to the citation, CDPH investigated a facility-reported injury of unknown origin to Resident 1, an 81-year-old woman whose decision-making was severely impaired. On June 28, 2026, a certified nursing assistant found her in bed bleeding from the face, with her right eye bulging. The department determined that Resident 2, a 74-year-old man, struck her in the face. A change-of-condition note that day recorded minor blood splatter on his hands.

CDPH found that Resident 2 had a known history of physical aggression. His care plan called for close visual monitoring and a one-to-one sitter (a staff member who stays within arm's reach or sight), and physician orders dated June 19 and June 22, 2026 required one-to-one supervision because of assaultive behavior and an earlier altercation. Inspectors found that surveillance video from June 28 showed Resident 2 leaving his room unsupervised at about 1:33 p.m., staying in Resident 1's room for about two minutes, and returning to his room unsupervised.

According to the citation, a hospital found a severe right eye injury with bleeding inside and around the eye, and a CT scan raised concern for a ruptured eyeball. Resident 1 had emergency eye surgery and stayed in the hospital from June 28 to July 1, 2026. She later told inspectors Resident 2 "beat her up" and that she worried about losing her eye and her sight. CDPH issued a Class A citation with a $20,000 penalty. The 2567 report states the federal survey cited F600 (freedom from abuse and neglect) at severity G.

September 2024: Class A citation and F697 (severity G) after nurses did not assess or treat two residents' pain

According to the citation, CDPH investigated a complaint and reviewed pain care for two residents. Resident 1 was a 65-year-old woman with a stage 4 pressure ulcer (a deep bed sore) on her tailbone. Her physician ordered pain checks every shift and during wound care, and Norco, a strong pain medicine, as needed. CDPH found the medication record did not show she was assessed for pain on July 18 and July 21, 2024. For Resident 3, who had a rib fracture and difficulty walking, the department determined that pain was not assessed on the evening and night shifts of July 15 and July 16, 2024.

Inspectors found that on July 30, 2024, Resident 1 was crying and said she felt helpless because she could not get her pain relieved. She said that on July 29 she asked for pain medicine for pain rated 10 out of 10 in her wound, and a licensed vocational nurse (LVN 1) did not give it. According to the citation, LVN 1 said Resident 1 had to wait until 9 p.m. because she had recently received a muscle relaxant.

CDPH found that both residents had pain for extended periods, and the facility did not follow its own policy that nurses give pain medicine as the physician ordered. According to the citation, Resident 3 had a decline in daily activities and walking and a heart rate of 127 on July 21, 2024, and was sent to an acute care hospital. CDPH issued a Class A citation with a $20,000 penalty; the facility appealed, and the final penalty was $14,000. The 2567 report states the federal survey cited F697 (pain management) at severity G.

May 2024: F600 (severity G) and F609 (severity D) after one CNA struck a resident with gloves and another held a resident's arms down

The 2567 report states that inspectors investigating a complaint in May 2024 found the facility did not keep two residents free from abuse. Resident 1 had dementia and severe cognitive impairment and used a wheelchair. According to the report, on May 21, 2024, CNA 1 pushed her wheelchair into her room after she made racial slurs in the hallway. CNA 1 said Resident 1 tried to kick her and that she blocked the kick with a hand holding latex gloves, which struck the resident's forearm.

Inspectors found that Registered Nurse 1 saw CNA 1 strike Resident 1's hand with the gloves, did not see the resident kick, and described the strike as a "purposeful movement" and not a reflex. Resident 1 told inspectors she had asked CNA 1 about her shoes, and CNA 1 hit her with the gloves and told her, "Go to bed." The administrator told inspectors there were no situations in which staff should strike a resident.

The 2567 report states that on May 7, 2024, CNA 4 held Resident 4's arms to restrain him while he was in bed. A change of condition report that day said he tried to punch and kick CNA 4. According to the report, Resident 4 had two skin tears on his right forearm and wrist with bleeding and pain rated 6 out of 10, and the report did not include a statement from Resident 4 or CNA 4 about how they happened. The federal survey cited F600 (freedom from abuse and neglect) at severity G and also lists F609 (reporting of alleged violations) at severity D.

September 2023: Class A citation and F689 (severity G) after a resident without an accessible call light fell twice and broke her forearm

According to the citation, Resident 1 was a 68-year-old woman with arthritis in many joints, scoliosis (a sideways curve of the spine) and muscle weakness. She needed total help with daily activities and could only steady herself with staff help when standing. CDPH found that her fall risk care plan from April 2023 called for keeping the call light within reach, prompt responses to requests for help, hourly safety checks and a low bed.

The department determined that on April 27, 2023, she was found kneeling on the floor beside her bed. According to the citation, she said she dropped her call light and fell while reaching for it. Her updated care plan again said to keep the call light within reach. Inspectors found that a June 30, 2023 note recorded her crawling out of her room to the nurses' station.

CDPH found that the facility did not keep her free from falls and did not make sure she had a call light she could reach to ask for help. On July 25, 2023 at about 11 a.m., she had an unwitnessed fall after telling nurses she was going to the restroom, with scrapes on her right knee and elbow and swelling of her right forearm. According to the citation, she had a new fracture of the distal radius (a forearm bone near the wrist) and needed a sling and a sugar-tong splint. CDPH issued a Class A citation with a $25,000 penalty. The 2567 report states the federal survey cited F689 (free of accident hazards, supervision and devices) at severity G.

CDPH substantiated at least one allegation in 97 of them. 132 intakes involved an allegation in CDPH’s abuse category, and 54 of those had an abuse allegation substantiated. An intake can contain several allegations; “unsubstantiated” means CDPH did not find enough evidence, not that nothing happened.

Sexual abuse reports at Vernon Healthcare Center

Cal Health Find lists 5 intakes for Vernon Healthcare Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (3 from complaints and 2 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 1 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. Staffing at Vernon Healthcare Center

Payroll records Vernon Healthcare Center filed with the federal Centers for Medicare & Medicaid Services (CMS) for April through June 2026 show an average of 4.33 nursing hours per resident per day, counting registered nurses (RNs), licensed vocational nurses and nurse aides. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 0.34 hours per resident per day (California average 0.68). Staffing averaged 3.83 hours on weekends and 4.53 on weekdays.

California requires most skilled nursing facilities to provide at least 3.5 direct care hours per patient day (Health and Safety Code section 1276.65). Counting the RN, LVN and nurse aide hours in the same payroll data, Vernon Healthcare Center was below 3.5 on 2 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

Vernon Healthcare Center lawsuits and criminal cases

We searched California Attorney General and Los Angeles County District Attorney announcements and news coverage for Vernon Healthcare Center and its licensee and found no public criminal case or civil lawsuit we could tie to this facility as of October 30, 2026. Many civil cases settle quietly or are not covered by the press, so this is not a complete court search.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
