What inspectors found at Western Convalescent Hospital
Tap a finding to read the summary. Each one is written from the government report linked under it.
April 2023: F880 (severity K) for infection control during a scabies outbreak
According to the 2567 report, Resident 1 was sent to a hospital with rashes on March 25, 2023, was diagnosed and treated for scabies (a contagious, very itchy skin condition caused by tiny mites) on March 26, and returned to the facility on April 4. Inspectors found the facility then failed to maintain an infection prevention and control program for scabies for the eight residents in the sample.
According to the report, the facility did not tell the dermatologist about Resident 1's diagnosis so that six residents with symptoms could be checked and treated, and did not address those residents' rashes. It did not give preventive treatment to Resident 2, Resident 1's roommate. CNA 1, who had not received preventive treatment, cared for two residents on contact isolation without personal protective equipment. Inspectors found the facility did not follow its own scabies policy, which placed 109 residents, staff, vendors and visitors at risk of exposure.
According to the report, the department identified an immediate jeopardy on April 25, 2023. On April 27, the administrator gave an acceptable removal plan, which listed dermatologist exams and permethrin (a scabies medicine) for 21 residents with rashes, preventive treatment for eight roommates, contact precautions, bagging of belongings, deep cleaning of 16 rooms, skin scrapings to test for scabies and one-on-one retraining for CNA 1. The facility was cited under F880, on infection prevention and control, at severity K, which means immediate jeopardy found in a pattern.
March 2023: F689 (severity G) after a resident fell from bed during care by one aide
According to the 2567 report, Resident 1 had contractures (permanent tightening) of both knees, dementia, seizures and a tracheostomy (a breathing opening in the neck), and depended on a ventilator. She could not make her own decisions and had involuntary movements. Her care plan said she was at risk of falling from her low air loss mattress and needed two-person help with transfers, repositioning and daily care, along with side rails.
Inspectors found CNA 1 cared for Resident 1 alone on March 5, 2023, instead of with a second person as the care plan required. According to the facility's follow-up report cited by inspectors, CNA 1 turned her onto her right side to finish cleaning her in bed, and she began shaking and jerking and slid off the bed to the floor. According to the report, hospital records said staff rolled her over without the side rails up. A family member told inspectors she had always seen two staff changing Resident 1.
According to the report, Resident 1 had a bump and a cut on the back of her head and was taken to a hospital, where the cut was closed with two staples. An x-ray of her middle spine showed multiple compression fractures, and the facility's follow-up report noted a spinal fracture. The facility was cited under F689, on accident hazards, supervision and devices, at severity G, meaning actual harm.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
