·
January 2026: Class AA citation and F880 (severity G) after Legionella bacteria in the water system went unaddressed

According to the citation, CDPH investigated in November 2025 after a complaint about a presumed healthcare-associated case of Legionnaires' disease, a severe pneumonia caused by Legionella bacteria that can grow in building water systems. CDPH found that reports from an outside water safety company, dated from February 2024 through March 2025, showed Legionella bacteria in the facility's water system starting in February 2024. The department determined the facility did not act on those findings or carry out the company's recommendations.

Inspectors found the facility did not review or follow its water management safety plan, the infection preventionist nurse did not oversee the infection control program in relation to the Legionella results, and the facility did not identify parts of the water system where the bacteria could grow. According to the citation, there was no process for the previous administrator to tell the current administrator, who started in April 2025, about the positive results. The 2567 report states the facility also did not report the positive water results to CDPH.

According to the citation, Resident 1, an 86-year-old man with chronic obstructive pulmonary disease (COPD) and diabetes who depended on staff for daily care, had a change in condition in late October 2025. He had a fever, shortness of breath and altered mental status, and was found unresponsive. At an acute care hospital, a sputum (mucus from the lungs) test detected Legionella bacteria. The citation states he died at the hospital six days later. CDPH issued a Class AA citation with a $120,000 penalty. The 2567 report states the federal deficiency was F880 (infection prevention and control) at severity G.

October 2025: Class A citation and F689 (severity G) after a resident with repeated falls was left alone and fell in a bathroom

According to the citation, Resident 5 was admitted in June 2025 with dementia, a past stroke and repeated falls. The 2567 report states she had moderately impaired thinking, needed help with toilet transfers and mobility, and fell eight times between late June and early August 2025. After her first fall in the facility, an assessment found her at high risk for falls.

CDPH found the facility did not add Resident 5 to its "Falling Star" fall prevention program until nearly a month after that assessment, did not hold the required team meeting after her first two falls, and did not provide the hourly rounds the program requires, including checking whether residents need to use the bathroom. Inspectors found that her physician ordered increased supervision and a wheelchair alarm in July 2025, but the alarm did not sound when she stood up from her wheelchair on the day of her August fall.

According to the citation, at about 6 p.m. that day, CNA 4 left Resident 5 alone in her wheelchair after she was given her dinner tray. She stood up and went into a bathroom without the alarm going off. The 2567 report states she was found on the toilet with a 2-centimeter cut and a 3-by-3-centimeter hematoma (a pocket of blood under the skin) on the back of her scalp, and was bleeding. LVN 6 applied a dressing, and she was sent to an acute care hospital. CDPH issued a Class A citation with a $25,000 penalty. The 2567 report states the federal deficiency was F689 (accident hazards, supervision and devices) at severity G.

May 2025: Class AA citation, F684 (severity J), F600 (severity J) and F580 (severity J) after staff did not act on a resident's low blood pressure

According to the citation, Resident 1, a 73-year-old man with high blood pressure, an underactive thyroid and a blocked neck artery, was admitted in March 2025. About a week later, at 5 p.m., his blood pressure was 86/57 and his heart rate was 111, both outside normal ranges. CNA 1 told inspectors she checked his vital signs four or five times, the readings were "consistently low," and she told LVN 1, who said she would recheck them. CDPH found no other blood pressure or heart rate readings were recorded after 5 p.m.

Inspectors found LVN 1 did not recheck and monitor his vital signs or report the change to the registered nurse on duty, and his physician was not notified. According to the 2567 report, later that night the physician called RN 1 several times before reaching him. RN 1 reported the same 5 p.m. readings and said it was unclear whether Resident 1 was responsive, and the physician ordered a transfer. Paramedics found Resident 1 in severe respiratory distress and took him to an acute care hospital, where he was pronounced dead that afternoon. The 2567 report states the preliminary cause of death was septic shock (very low blood pressure caused by an infection).

CDPH issued a Class AA citation with a $120,000 penalty. The 2567 report lists F684 (quality of care), F600 (freedom from abuse and neglect) and F580 (notification of changes), all at severity J, and inspectors called an Immediate Jeopardy. The 2567 report states the removal plan included checking vital signs for all 262 residents, notifying physicians about 45 residents with recent changes, a reporting protocol and log for abnormal vital signs, and a root cause analysis.

February 2025: F600 (severity G) after a CNA was seen hitting a resident on the head

The 2567 report states that in late January 2025, two student nurses saw CNA 1 hit Resident 1 on the back of the head three times with an open hand. Resident 1 had been admitted in November 2023 with paralysis and weakness on his right side after a stroke, heart failure and difficulty walking, and had moderately impaired thinking.

Inspectors found that one student nurse wrote that Resident 1 and CNA 1 had been arguing in the bathroom, that Resident 1 yelled in Spanish that the CNA hit him, and that CNA 1 said, "I hit him." The other student nurse told inspectors CNA 1 slapped the back of his head three times very hard. According to the 2567 report, Resident 1 told inspectors he had asked for help, CNA 1 did not want to help him and started hitting him, and he felt sad and ashamed and had pain in the back of his head. He said he could not defend himself because he was sick.

The 2567 report states CNA 1 was removed right away, a head-to-toe check found no skin injuries, and Resident 1 was seen for a psychiatric follow-up. The director of nursing told inspectors the incident was considered abuse and that CNA 1 was taken off the staffing schedule. The 2567 report states the federal deficiency was F600 (freedom from abuse and neglect) at severity G.

Sexual abuse reports at Mirage Post Acute

Cal Health Find lists 18 intakes for Mirage Post Acute from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (9 from complaints and 9 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. Staffing at Mirage Post Acute

Payroll records Mirage Post Acute filed with the federal Centers for Medicare & Medicaid Services (CMS) for April through June 2026 show an average of 4.32 nursing hours per resident per day, counting registered nurses (RNs), licensed vocational nurses and nurse aides. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 0.35 hours per resident per day (California average 0.68). Counting the RN, LVN and nurse aide hours in the same payroll data, Mirage Post Acute was below 3.5 on 0 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

Mirage Post Acute lawsuits and criminal cases

We searched California Attorney General and Los Angeles County District Attorney announcements and news coverage for Mirage Post Acute and its licensee and found no public criminal case or civil lawsuit we could tie to this facility as of October 30, 2026. Many civil cases settle quietly or are not covered by the press, so this is not a complete court search.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
