December 2025: Class A citation, F689 (severity G) and F609 (severity D) after an unwitnessed fall with a broken arm

CDPH investigated a complaint in November 2025 about a fall with injury. According to the citation, Resident 1, a 94-year-old woman admitted in August 2025 with a hip fracture, had dementia, a history of falls and a physician's order for a bed pad alarm (a sensor that sounds when a resident gets up). Her family had hired a companion to sit with her. On the evening of August 26, 2025, the companion told LVN 3 that the resident kept trying to get out of bed.

Inspectors found LVN 3 told the companion to watch the resident, did not check the pad alarm, did not ask a CNA to help and did not tell the registered nurse before continuing his medication pass. The companion stepped out to make a phone call, and at 9:21 p.m. the resident had an unwitnessed fall. Staff found her face down with the alarm unplugged; the director of nursing said CNA 3 had left it off after giving care. LVN 3 and two CNAs moved her back to bed before a registered nurse assessed her, which facility procedure does not allow. At an acute care hospital she was diagnosed with a displaced fracture of the upper arm near the elbow that needed surgery with metal hardware, plus head trauma and swelling around the left eye.

CDPH issued a Class A citation with a $25,000 penalty. The federal 2567 report cited F689 (accident hazards and supervision) at severity G. The report also cited F609 at severity D because the facility did not report an allegation of staff-to-resident abuse involving a second resident to the ombudsman, as its abuse policy requires.

January 2025: Class AA citation and F580 (severity J) after a resident's low blood pressure was not reported

CDPH visited the facility on November 16, 2024, to investigate a complaint about a resident's death. According to the citation, Resident 1, a 48-year-old man with a traumatic brain injury, was on a ventilator, had a tracheostomy (a breathing tube through the neck) and received tube feeding. He had a diagnosis of low blood pressure on admission, but the department found the facility did not develop and implement a care plan for it.

Inspectors found that at 1:00 a.m. on November 3, 2024, CNA 1 reported a blood pressure of 89/54, down from 118/75 the evening before. LVN 1 told inspectors she raised his feet and got a normal reading, but she did not complete a change of condition form, did not tell the registered nurse or the physician and did not monitor his blood pressure again until the end of her shift. Two physicians said they were not told of the 1:00 a.m. reading, and one said vital signs should have been checked every 15 to 30 minutes. At about 6:17 a.m. a respiratory therapist found dark brown, coffee-ground discharge while suctioning his airway. He later had blood in his stool, and by 10:15 a.m. his blood pressure was 38/28. He was sent to an acute care hospital, where, according to the citation, he died on November 13, 2024.

CDPH issued a Class AA citation and assessed a $120,000 penalty, which the facility appealed. The 2567 report cited F580 (notification of changes) at severity J and states the facility's immediate jeopardy removal plan included re-educating licensed nurses on documenting and reporting changes in condition and abnormal blood pressure readings.

March 2023: Class A citation and F686 (severity G) after a pressure injury worsened to stage 4

During a January 2023 recertification survey, CDPH found the facility did not give a resident the care needed to prevent a pressure injury (bed sore) from forming and getting worse. According to the citation, Resident 93, a 70-year-old woman with a traumatic brain injury and diabetes, had no wound on her sacrococcyx (the area at the base of the spine and tailbone) when admitted in 2021. A stage 3 pressure injury there was recorded in September 2022. She depended on two staff for bed mobility.

Inspectors found her wound treatment order ran out in late December 2022 and no one called the physician for a new one, so she received no wound treatment from December 27, 2022, until January 2, 2023. Weekly wound assessments were missed on December 30 and January 6, and CNAs did not do skin checks on four shower days. The director of nursing said no one had been auditing the weekly assessments. By January 16, 2023, the wound measured 8.5 by 8.5 centimeters and 4 centimeters deep with exposed muscle and tendon, and it was reclassified as stage 4. The wound nurse later found a fever, odor and signs of infection. The registered dietitian told inspectors she was never notified that the wound had worsened.

CDPH issued a Class A citation with a $25,000 penalty, finding the resident developed a facility-acquired stage 3 pressure injury that progressed to stage 4. The federal 2567 report cited F686 (treatment and services to prevent and heal pressure ulcers) at severity G.

Sexual abuse reports at The Ellison John Transitional Care Center

Cal Health Find lists 10 intakes for The Ellison John Transitional Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 9 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 1 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 0.62 hours per resident per day (California average 0.68). Counting the RN, LVN and nurse aide hours in the same payroll data, The Ellison John Transitional Care Center was below 3.5 on 0 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
