December 2024: Class A citation and F684 (severity G) after a resident's breast lump went without follow-up for months
CDPH made an unannounced visit on November 5, 2024, to investigate a complaint about quality of care, resident rights and administration. According to the citation, Resident 2 was a 68-year-old woman with multiple sclerosis, cerebral palsy and hydronephrosis (swelling of a kidney from a buildup of urine). On April 17, 2024, a licensed vocational nurse noted a lump in her left breast after a shower, and the physician recommended a mammogram of both breasts.
CDPH determined the facility failed to make sure LVN 1 promptly notified the physician, and that licensed nurses developed a plan of care for the lump. According to the citation, several LVNs did not clarify the physician's mammogram recommendation, and nurses did not reassess the lump after 72 hours or follow up with the physician for orders. The department found a mammogram order was written on April 25, 2024, but was not carried out as the physician ordered. CDPH found that, as a result, Resident 2 did not receive care for the lump from April 17 to July 1, 2024.
According to the citation, on July 1, 2024, Resident 2 reported severe, uncontrolled pain under her left breast and left rib cage and was transferred to an acute care hospital. The hospital found a mass of about 1.8 by 2.4 by 1.3 centimeters that was suspicious for cancer. The citation states a biopsy on July 31, 2024, showed infiltrating ductal carcinoma, a type of breast cancer that starts in the milk ducts and spreads into nearby tissue. CDPH issued a Class A citation with a $25,000 penalty, and the related survey cited F684, which covers quality of care, at severity G.
March 2023: F697 (severity G) over unrelieved leg pain in a resident wearing knee splints
Inspectors found during a March 2023 complaint investigation that the facility did not manage pain for Resident 3. According to the 2567 report, Resident 3 had contractures (shortened, stiff muscles and tendons) in both knees, severe cognitive impairment and unclear speech, and received medication through a feeding tube. His care plan called for staff to respond right away to any complaint of pain and to check whether pain treatments worked.
The 2567 report states that on March 16, 2023, at 2:28 p.m., an inspector saw Resident 3 in bed crying, screaming, sweating and grimacing, with tears on his face, biting his fist and later hitting his face with his hand. A restorative nursing assistant told inspectors she had put splints on both of his legs at 2 p.m., that the splints were too painful for him, and that he could not tolerate them. His roommate told inspectors he cried in pain even at night. Inspectors found LVN 1 had given Tylenol at 2 p.m. but did not assess his leg pain or check whether the medicine helped, as his care plan required.
According to the report, the medication record for March 16 showed no pain and no yelling or moaning on any shift. Inspectors found no documented evidence that LVN 1 notified the physician about his unrelieved pain, as the facility's own policy required. The report states Resident 3 had physical and emotional distress from uncontrolled pain in both legs on March 16 and 17, 2023. Inspectors cited F697, which covers pain management, at severity G.

Arbor Glen Care Center lawsuits and criminal cases
We searched California Attorney General and Los Angeles County District Attorney announcements and news coverage for Arbor Glen Care Center and its licensee and found no public criminal case or civil lawsuit we could tie to this facility as of October 30, 2026. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
