September 2025: Class A citation after nurses did not notify a physician of a resident's fall and back pain

According to the citation, CDPH made an unannounced visit on July 21, 2025 to investigate a facility-reported injury of unknown origin. CDPH found that Resident 1, a 78-year-old woman with dementia and metabolic encephalopathy (brain dysfunction caused by an underlying medical problem), was at high risk for falls and depended on staff for most daily activities. On July 16, 2025, at 11 p.m., she had an unwitnessed fall in her bathroom and later complained of back pain.

Inspectors found that CNA 1 told LVN 1 about the fall that night, and that LVN 1, RN 1 and RN 2 each learned of the fall or her pain on July 16 or 17, yet none of them notified her physician as the facility's change-in-condition policy required. RN 1 told inspectors that LVN 1 was asked to fill out a change-in-condition form but did not, and that RN 1 also did not fill one out or call the physician. The department determined the facility delayed her treatment until July 18. Early that morning LVN 1 notified the physician, who ordered an immediate X-ray of her back. The X-ray showed an acute L1 compression fracture (a break in the first bone of the lower spine), and she was sent to an acute care hospital that night, where a CT scan showed a nondisplaced fracture of the L1 vertebra.

According to the citation, the director of nursing told inspectors the licensed nurses did not follow the facility's policy when they did not tell the physician about the fall and the pain. CDPH issued a Class A citation with a $25,000 penalty.

April 2025: Class A citation and F689 (severity G) and F609 (severity D) after a resident's head injury and a fight with his roommate

According to the citation, CDPH conducted an unannounced recertification survey on March 4, 2025. CDPH found that Resident 37, a 55-year-old man with dementia, had a care plan for banging his head and punching the wall. The plan called for nursing staff to anticipate his needs and give him positive interaction and attention, and his physician had ordered hourly monitoring for aggressive behavior.

Inspectors found that licensed vocational nurses did not carry out that care plan and CNAs did not provide the ordered hourly monitoring. On March 2, 2025, he threw a remote control to the floor in the hallway, turned and hit the top of his head on a door. He was taken to an acute care hospital with a head contusion (bruise) and a scalp cut that was closed with skin adhesive glue. According to the citation, on March 4 his roommate, Resident 196, punched him in the face and head. He had a bloody nose and mouth, and a CT scan at the hospital showed a mildly displaced fracture of the nasal septum (the bone between the nostrils) and a hematoma (pool of blood) on his forehead.

The 2567 report states the facility also left a lunch tray in the room of another resident, Resident 294, who depended on staff for eating, before staff were ready to help him, and he fell while reaching for it. CDPH issued a Class A citation with a $25,000 penalty. The federal report cites F689 (free of accident hazards, supervision and devices) at severity G and F609 (reporting of alleged violations) at severity D; the full narrative on the reporting finding is in the linked report.

March 2025: F600 (severity G) after a resident hit and scratched a roommate, who suffered a broken nose

The 2567 report states that CNA 1 and LVN 1 failed to ensure Resident 4 was not physically assaulted by another resident, Resident 5, on March 20, 2025. Inspectors found that Resident 4, who had dementia and moderately impaired thinking, walked to the bed of roommate Resident 5 and began making it while Resident 5 was asleep. Resident 5 got up, screamed, hit Resident 4 on the head with a fist and scratched Resident 4's face. Resident 4 grabbed Resident 5's hair.

Inspectors found that staff heard screaming from the room and LVN 1 separated the two residents. Resident 4 had a bump on the forehead, finger scratch marks on the face, bleeding from the right nostril and face pain rated 5 out of 10. At an acute care hospital, a CT scan showed a mildly comminuted (broken into pieces), minimally displaced fracture of the right nasal bone, and the diagnosis was a closed head injury and nasal fracture. Resident 5 also had scratches and a bleeding nose and was transferred to a hospital for evaluation the next day. Four days later, Resident 4 could not recall the altercation and reported nose pain of 4 out of 10.

CDPH found the facility failed to protect Resident 4 from physical abuse. CNA 1 told inspectors that residents with aggressive behavior should be monitored closely to prevent altercations. The 2567 report cites F600 (free from abuse and neglect) at severity G.

September 2024: Class A citation and F600 (severity H) after two resident-to-resident assaults

According to the citation, CDPH made an unannounced visit on July 31, 2024 to investigate a facility-reported incident of resident abuse. CDPH found the facility failed to protect two residents from physical abuse by other residents. On July 24, 2024, Resident 2, a 45-year-old man whose care plans noted striking out and verbal aggression, punched Resident 3, a 62-year-old man, in the face. On July 28, Resident 5 hit Resident 6 on the head with a water pitcher.

Inspectors found that the first incident happened on an outdoor patio at about 9:30 p.m. A resident who saw it told inspectors that Resident 2 placed a towel over a light fixture, Resident 3 told him to stop because it could cause a fire, and after the two exchanged words Resident 2 pushed a metal table toward Resident 3 and punched him. The witness said no staff were present to stop it and that staff came after it was over. According to the citation, a CT scan at an acute care hospital showed fractures of the right cheekbone area, the walls of the right maxillary sinus and the floor of the right eye socket, along with bruising of the face. Resident 3 told inspectors he had severe pain and felt angry. Resident 6 had a cut on the left forehead that needed stitches at a hospital.

According to the citation, CDPH issued a Class A citation with a $25,000 penalty. The federal 2567 report cites F600 (free from abuse and neglect) at severity H.

Sexual abuse reports at Mesa Glen Care Center

Cal Health Find lists 7 intakes for Mesa Glen Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 6 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
