August 2026: Class A citation and F687 (severity G) after a resident with diabetes developed two foot ulcers
According to the citation, CDPH found the problem during its annual recertification survey, which began with an unannounced visit on July 13, 2026. Resident 10 was admitted in May 2026 with type 2 diabetes and diabetic polyneuropathy (nerve damage that can cause numbness in the feet), and the care plan called for staff to check the resident's skin during daily care. Inspectors found no documented skin checks from June 15 to July 15, 2026, and the task had been set to "as necessary" instead of daily.
CDPH found that on June 26, 2026, a podiatry nurse practitioner found a diabetic ulcer (an open sore) on the outer side of the left foot and recommended daily foot inspections, daily washing and pressure offloading with heel protectors. The treatment nurse told inspectors she did not read that note and did not carry out the recommendations, and a CNA stated the resident's feet were washed only on shower days. On July 9, a wound care consultant found the left foot ulcer had doubled in width and was mostly covered with dead tissue, and found a new ulcer on the right big toe. The director of nursing stated the ulcers were avoidable because the recommendations and the facility's diabetic foot care policy were not followed.
CDPH issued a Class A citation with a $20,000 penalty, and the department determined the violations presented either imminent danger or a substantial probability of death or serious physical harm. The federal 2567 report for the same survey cited F687 (Foot Care) at severity G.
July 2024: F600 (severity G) after one resident hit another in the face near the nursing station
According to the 2567 report, inspectors found the facility failed to protect Resident 2 from physical abuse by another resident, Resident 1, on July 3, 2024. Both residents had severely impaired cognition, according to their assessments. A licensed vocational nurse told inspectors she saw Resident 1 walk past Resident 2 in front of the nursing station, heard a disruption, and turned to see the two "struggling" and "grabbing each other's arms." She separated them and noticed discoloration around Resident 2's left eye.
The 2567 report states that Resident 2 had a cut about 1.5 centimeters wide near the left eye, with bleeding, redness and swelling, and needed ice packs and daily wound care with an antibiotic ointment for 14 days. When inspectors visited on July 19, 2024, they saw purplish discoloration and swelling still visible around his left eye. The director of nursing told inspectors the injury was consistent with being hit, that it was physical abuse that should not have been allowed to happen, and that the facility did not follow its abuse prevention policy. The administrator in training, who served as the facility's abuse coordinator, also stated the incident was physical abuse. Inspectors found the facility had not protected Resident 2's right to be free from abuse and cited F600 (Free from Abuse and Neglect) at severity G.

CDPH substantiated at least one allegation in 12 of them. 23 intakes involved an allegation in CDPH’s abuse category, and 2 of those had an abuse allegation substantiated. Cal Health Find lists 7 intakes for Stoney Point Healthcare Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (6 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

|---|---|---|
| 2024 | 2 | 0 |

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
