·
April 2024: F600 (severity G) after a resident pulled another resident's hair and she fell

Inspectors conducted a certification survey at the facility in April 2024. The 2567 report states that at about 7:55 a.m. on April 7, 2024, Resident 106 went toward Resident 100, grabbed her and pulled her by the hair, and she fell to the floor. Resident 100 told inspectors Resident 106 pulled her hair and pulled her out of her wheelchair onto the floor. Resident 100 had intact cognition, and Resident 106 had severely impaired cognition.

According to the report, Resident 100 had slight bleeding on her scalp from Resident 106's fingernails and pain in her left ankle. She told inspectors she was "shaken" by the incident and that "it was scary." A licensed vocational nurse and a registered nurse told inspectors the incident was physical abuse. The director of nursing said it could possibly have been prevented if staff had taken Resident 106 back to that resident's room, and the administrator and director of nursing both said the hair pulling was deliberate.

Inspectors found the facility failed to protect Resident 100's right to be free from physical abuse by another resident. According to the 2567 report, the facility's own abuse prevention policy defined abuse as the willful infliction of injury, with willful meaning the person acted deliberately. The survey cited F600 (free from abuse and neglect) at severity G.

Sexual abuse reports at Chatsworth Park Health Care Center

Cal Health Find lists 6 intakes for Chatsworth Park Health Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (3 from complaints and 3 reported by the facility itself). Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
