June 2023: Class A citation and F689 (severity G) after a resident fell from the wrong type of lift and fractured his thigh bone
According to the citation, CDPH received an anonymous complaint about resident safety and falls and made an unannounced visit on May 11, 2023. Resident 1, a 41-year-old man with complete paraplegia (paralysis of the legs) and a below-the-knee amputation of the right leg, was assessed as needing two or more staff for transfers and a full body mechanical lift to move between surfaces. CDPH found that on December 29, 2022, CNA 1, a temporary worker from a staffing registry, moved him from a shower chair to bed alone using a sit-to-stand lift, a device meant for residents who can bear weight on their legs.
According to the citation, the resident told inspectors he asked not to use that lift, felt dizzy as it moved, and then the lift went down, his left leg bent under him and he heard a loud pop. He said no staff checked on him the rest of that day. A CNA noticed swelling of his left thigh the next day, and an X-ray on January 1, 2023, showed a spiral fracture of the left femur (thigh bone). CNA 1 told inspectors she never received training from the facility on the lifts. The director of staff development stated she did not orient registry staff on the lifts unless they asked, even though facility policy required training and a demonstration of skill.
CDPH issued a Class A citation with a $25,000 penalty, and the department determined the violations presented either imminent danger or a substantial probability of death or serious physical harm. The federal 2567 report for the same investigation cited F689 (Free of Accident Hazards/Supervision/Devices) at severity G.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
