February 2025: Class AA citation and F757 (severity K) over blood pressure and pulse checks for a resident's heart medications

According to the citation, CDPH investigated a complaint involving Resident 1, who was admitted in October 2024 with heart failure, atrial fibrillation (an irregular heartbeat) and a history of a heart attack. Her physician ordered amiodarone, metoprolol, spironolactone and Entresto, each to be held if her blood pressure or pulse fell below set limits. CDPH found that in November and December 2024 nurses repeatedly held doses without recording her blood pressure or pulse, gave doses when her blood pressure was below the limits, and did not document the apical pulse (taken over the heart) ordered for amiodarone. Her physician told inspectors she was not told the medications were held or given outside the limits.

Inspectors found the electronic record had flagged a severe interaction between amiodarone and metoprolol that can cause a very slow heart rate, low blood pressure and cardiac arrest. The director of nursing acknowledged she had cleared one of the warnings and had not notified a physician. On December 9, 2024, Resident 1 received all four heart medications in the morning, and about 1 p.m. a second lidocaine pain patch with no record that the first was removed. A licensed vocational nurse stated she did not recheck vital signs before the resident left at 1:20 p.m. Staff at the assisted living facility said she showed no sign of life when she arrived, and she was pronounced dead that afternoon.

CDPH issued a Class AA citation and assessed a $120,000 penalty, which the facility has appealed. CDPH determined the violations were a substantial factor in her death. The 2567 report cited F757 (Drug Regimen is Free from Unnecessary Drugs) at severity K for the same complaint.

April 2023: F695 (severity G) after the facility had no nonrebreather oxygen mask for a resident with low oxygen levels

The 2567 report states that inspectors investigated the care of Resident 1, who was admitted in February 2023 with acute respiratory failure and congestive heart failure. On April 24, 2023, at about 5:23 p.m., she became short of breath and her oxygen saturation (the amount of oxygen in the blood) was 79 to 83 percent on room air. A licensed vocational nurse gave oxygen through a nasal cannula and then a simple face mask, but her level did not rise above 90 percent, and the nurse called 911 at 5:30 p.m.

Inspectors found the facility did not have a nonrebreather mask, a device used to deliver a high, constant flow of oxygen in an emergency. A paramedic told inspectors that when he asked for one, the nurse showed him a bag valve mask (a hand-held breathing bag) instead. Paramedics arrived at 6:09 p.m., found her oxygen level at 88 percent and placed their own nonrebreather mask, and her level rose to 96 percent. She was taken to a hospital, where a chest X-ray showed fluid or other material in the lungs. The director of nursing confirmed the facility had no nonrebreather masks at the time and said they were delivered within 24 hours after the incident.

The 2567 report states the facility did not follow its own emergency supplies policy, which called for medical supplies to be kept in adequate amounts and easy to reach. Inspectors found the missing equipment delayed a lifesaving intervention until paramedics arrived, and cited F695 (Respiratory/Tracheostomy Care and Suctioning) at severity G.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
