March 2026: F600 (severity G) after one resident struck another in the face
Inspectors investigating a facility-reported incident found that on March 19, 2026, at 10:55 a.m., Resident 4 stood up from a wheelchair on the patio and hit Resident 3 in the right eye area. According to the 2567 report, Resident 3 was sitting in a patio chair talking with other residents. An activity staff member who was present tried to step in, but Resident 4 had already struck Resident 3.
The report states Resident 3 had a cut above the right eye, measured at 0.5 to 1 centimeter long, purplish bruising and pain rated 2 out of 10. Staff gave first aid, and the cut was covered with steri-strips (adhesive strips that help close a cut). Resident 3 told inspectors he was shocked because he considered Resident 4 a friend, and said, "I wish it didn't happen." Inspectors noted both residents had intact cognition. According to the report, the facility transferred Resident 4 to another facility the next day.
Inspectors cited F600, the right to be free from abuse and neglect, at severity G, finding the facility failed to protect Resident 3 from physical abuse. The administrator told inspectors that willful contact between residents without consent is abuse and is never to be considered unavoidable.
February 2026: F812 (severity K) over fish thawed without time and temperature checks
During a recertification survey in February 2026, inspectors observed a pan of fish, not sealed in plastic, thawing under slow running water in the kitchen's meat preparation sink. According to the 2567 report, the cook said he started thawing it around 6 a.m. and took it out around 9 a.m. without checking its temperature. Inspectors and the dietary supervisor measured five pieces at 63.9 to 69.6 degrees Fahrenheit, inside the "danger zone" where bacteria grow quickly.
Inspectors found the kitchen kept no time or temperature logs for thawing and the dietary supervisor said he had not trained staff on thawing in the sink. The report also lists a chipped, peeling refrigerator rack, a rusty freezer rack, dented cans stored with other cans, dust in dry storage and trays stacked while wet. The report states 79 of 85 residents were at risk of foodborne illness.
Inspectors cited F812, safe food storage, preparation and service, at severity K, and called an Immediate Jeopardy on February 10, 2026. According to the report, the facility's removal plan stated the fish was thrown out and no resident was served it, and it added a thawing temperature log, retraining and testing for dietary staff, weekly checks and review by the quality committee. Inspectors lifted the Immediate Jeopardy on February 12, 2026.
September 2023: F600 (severity G) after a resident threw a trash bin at his roommate
Inspectors investigating a facility-reported incident found that on September 5, 2023, around 7:30 a.m., Resident 2 threw a plastic trash bin at his roommate, Resident 1. According to the 2567 report, Resident 1 had moderately impaired cognition and Resident 2 had intact cognition. Resident 2 told a registered nurse he lifted the trash can and threw it toward Resident 1. He told a licensed vocational nurse he wanted Resident 1 away from his closet and did not mean to hit him.
The report states Resident 1 had a skin tear about one centimeter long near his right eye with minimal bleeding, followed two days later by purple discoloration around the eye. A CNA who heard screaming stepped between the two residents, a licensed vocational nurse removed Resident 2 from the room, and another nurse gave first aid. A physician ordered daily wound care for 21 days and monitoring of the bruise. Resident 1 could not recall the incident when inspectors spoke with him.
Inspectors cited F600, the right to be free from abuse and neglect, at severity G, finding the facility failed to protect Resident 1 from physical abuse. The administrator told inspectors her investigation found Resident 2 threw the bin deliberately, which met the definition of abuse, and that Resident 2 expressed remorse afterward.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
