What inspectors found at All Saints Healthcare

Tap a finding to read the summary. Each one is written from the government report linked under it.

·
March 2026: Class A citation and F686 (severity G) after a resident developed a stage 2 pressure ulcer

According to the citation, CDPH identified during a survey that Resident 18 had moisture-associated skin damage (MASD, skin irritation or breakdown from long contact with urine, stool or sweat) on the left buttock when readmitted on October 28, 2025. Resident 18 had a history of bleeding in the brain, a stroke and chronic respiratory failure, had severely impaired cognition, depended on staff to turn in bed and was incontinent. Assessments rated Resident 18 at risk for pressure ulcers (bed sores).

CDPH found the facility failed to make sure licensed nurses and treatment nurses evaluated and documented the skin damage at least weekly. Inspectors found Treatment Nurse 1 did not report the wounds to the resident's physician or to the wound care physician at readmission and did not obtain treatment orders, and CNAs did not reposition Resident 18 every two hours. According to the citation, the facility also did not follow its own decubitus ulcer prevention policy. The wound care physician identified a facility-acquired pressure ulcer on January 20, 2026, and later that month found it had become a stage 2 pressure ulcer (a shallow open sore with partial loss of skin) on the left buttock.

The department issued a Class A citation for patient care with a $15,000 penalty. The federal 2567 report from the same survey cited F686, the rule on preventing and treating pressure ulcers, at severity G, which means actual harm. That report states inspectors also found pressure ulcer prevention problems for four other residents, including missed wound treatments, air mattresses not set to the resident's weight and heel protectors that were not applied.

December 2025: Class A citation and F689 (severity J) after a two-year-old resident fell from an adult-sized shower bed

According to the citation, CDPH made an unannounced visit on October 31, 2025 to investigate a facility-reported fall. Resident 1 was a two-year-old boy with chronic respiratory failure who used a tracheostomy and a ventilator, had a liver transplant and a feeding tube, could not communicate his needs and depended on staff for all daily care. His care plan said staff would not leave him unattended with side rails down.

CDPH found that at 8:15 a.m. on October 24, 2025, CNA 1 moved him from his crib to an adult-sized shower bed whose side rails had gaps 8 inches high and 22.5 inches wide, then turned her back and left him unattended. According to the citation, he fell 28 inches to the floor on his right side and had a 1-centimeter discoloration on his right cheek. Inspectors found the facility had not assessed whether an adult shower bed was safe for a child, did not use a pediatric shower bed, did not update his fall risk assessment after the fall, and did not follow the equipment manual, which said a caregiver should be present at all times.

The department issued a Class A citation for patient care with a $20,000 penalty. The federal 2567 report cited F689, the rule on accident hazards and supervision, at severity J, the immediate jeopardy level. According to that report, the state survey agency called an immediate jeopardy on November 6, 2025 and removed it on November 8 after verifying the facility's plan, which included retraining staff never to leave pediatric residents unattended during bathing and buying pediatric shower beds.

October 2023: F759 and F760 (severity K) after a 42.3 percent medication error rate during tube feeding medication passes

The 2567 report states that on October 18, 2023, inspectors observed medication administration and found 11 errors out of 26 opportunities, a medication error rate of 42.3 percent. The errors involved Residents 22, 43 and 68, who received medications through a gastrostomy tube (a feeding tube placed into the stomach), and were made by LVNs 1, 2 and 3.

According to the report, LVN 1 mixed three of Resident 22's medications together, including two seizure medications (Keppra and Dilantin) and potassium chloride, instead of giving them one at a time as facility policy required. Inspectors found LVN 2 did not flush Resident 43's tube with water before and between medications, including the blood thinner Xarelto, and did not let them flow by gravity. LVN 3 left significant undissolved residue of crushed medications in the cup, so Resident 68 did not get a full dose of Robinul, which reduces saliva and airway secretions. The report states these errors put the residents at risk of more seizures, irregular heartbeat, aspiration pneumonia, shortness of breath, hospitalization or death.

The 2567 report cited F759 (medication error rate of 5 percent or more) and F760 (significant medication errors), both at severity K, the immediate jeopardy level for a pattern. According to the report, the state survey agency called an immediate jeopardy on October 18 and removed it on October 20 after verifying the facility's plan. The plan included retraining LVNs 1, 2 and 3 before they returned to duty, checking residents they had medicated in the past seven days for side effects, and re-educating all licensed nurses on giving medications by tube.

CDPH substantiated at least one allegation in 14 of them. 11 intakes involved an allegation in CDPH’s abuse category, and 1 of those had an abuse allegation substantiated. An intake can contain several allegations; “unsubstantiated” means CDPH did not find enough evidence, not that nothing happened.

Sexual abuse reports at All Saints Healthcare

Cal Health Find lists 4 intakes for All Saints Healthcare from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 3 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 2.44 hours per resident per day (California average 0.68). Counting the RN, LVN and nurse aide hours in the same payroll data, All Saints Healthcare was below 3.5 on 0 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
