May 2026: Class A citation and F689 (severity G) after a resident fell during a transfer and fractured both thigh bones

During an annual recertification survey that began April 27, 2026, CDPH found that a certified nursing assistant did not give Resident 11 the two-person help her care plan required for transfers. According to the citation, Resident 11 was a 62-year-old woman with type 2 diabetes and end stage kidney disease who received dialysis. Her care plan required two staff and a Hoyer lift (a mechanical lift) for transfers.

Inspectors found that at 6:50 a.m. on April 20, 2026, CNA 7 moved Resident 11 from her bed to a wheelchair without a second staff member or the Hoyer lift, and did not follow the facility's fall management interventions for a resident at high risk for falls. According to the 2567 report, she slipped from the bed and landed on her knees. Three people put her back in the wheelchair using a bed sheet, she was given Tylenol, and she was taken to the lobby to wait for her ride to dialysis.

According to the citation, Resident 11 had severe knee pain and could not bear weight. She was sent to a hospital emergency room, where x-rays showed fractures of the lower thigh bone (femur) in both legs, one of them next to an existing orthopedic implant. She had surgery on her right leg on April 23 and on her left leg on April 30, 2026. CDPH issued a Class A citation with a $25,000 penalty, and the federal survey cited F689 at severity G, meaning actual harm.

January 2026: Class A citation over pressure injury and incontinence care for a resident

On November 25, 2025, CDPH made an unannounced visit to investigate an allegation about a resident's death. According to the citation, CDPH determined the facility failed to give Resident 1 skin and pressure injury care (care to prevent damage to skin and tissue from prolonged pressure, also called bed sores) that met professional standards and physician orders.

CDPH found the facility did not carry out an October 21, 2025, recommendation from a wound care specialist for pressure-relieving devices, and so did not prevent the development and worsening of Resident 1's pressure injuries. According to the citation, the facility also failed to provide incontinence care (care for the involuntary loss of urine or stool) to prevent moisture-associated skin damage, which is skin breakdown caused by prolonged contact with moisture.

According to the citation, the violations also involve rules on nutrition and hydration, intravenous fluids, accurate resident assessments, care planning, good personal hygiene and notifying the physician of changes in a resident's condition. The available excerpt of the citation ends before those findings and does not describe what happened to Resident 1 or the outcome of the investigation into the allegation. CDPH issued a Class A citation with a $25,000 penalty. The full narrative is in the linked report.

November 2023: Class A citation, F600 (severity J) and F658 (severity G) after staff held a resident down for a urine catheter

On September 29, 2023, CDPH made an unannounced visit to investigate a complaint about residents' right to be free from abuse. According to the citation, Resident 1 was an 88-year-old woman who was cognitively intact. A physician had ordered a urine test on July 5, 2023.

CDPH found that at 4:33 a.m. on July 6, 2023, LVN 1, CNA 1 and CNA 2 held Resident 1's arms and legs and spread her legs against her wishes, after she screamed at them to stop, while LVN 1 inserted a straight catheter (a tube placed in the bladder to collect urine). According to the citation, LVN 1 did not explain the procedure or get her consent. CDPH also found the facility catheterized Resident 1, Resident 2 and Resident 3 without a physician order for that procedure and did not document it. Resident 1 said she felt scared and had pain and emotional distress, and Resident 2 said he felt pain.

According to the citation, Resident 1 reported what happened on July 19, 2023. The 2567 report states inspectors called an immediate jeopardy on October 4 and removed it on October 6, 2023, after accepting the facility's removal plan, which included suspending and retraining the three employees and training staff on honoring residents' wishes. The federal survey cited F600 at severity J and F658 at severity G. CDPH issued a Class A citation with a $25,000 penalty, which CDPH records show was reduced to $22,500 after an appeal.

Cal Health Find lists 209 complaint and facility-reported incident intakes for this facility from 2023 through October 30, 2026. CDPH substantiated at least one allegation in 47 of them. 92 intakes involved an allegation in CDPH’s abuse category, and 9 of those had an abuse allegation substantiated. An intake can contain several allegations; “unsubstantiated” means CDPH did not find enough evidence, not that nothing happened.

Sexual abuse reports at Sharon Care Center

Cal Health Find lists 9 intakes for Sharon Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (5 from complaints and 4 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 1 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. Staffing at Sharon Care Center

Payroll records Sharon Care Center filed with the federal Centers for Medicare & Medicaid Services (CMS) for April through June 2026 show an average of 3.96 nursing hours per resident per day, counting registered nurses (RNs), licensed vocational nurses and nurse aides. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 0.66 hours per resident per day (California average 0.68). Staffing averaged 3.62 hours on weekends and 4.10 on weekdays.

CMS reports nursing staff turnover of 50.9% at this facility (California average 36.6%) and RN turnover of 50.0% (California average 37.8%).

California requires most skilled nursing facilities to provide at least 3.5 direct care hours per patient day (Health and Safety Code section 1276.65). Counting the RN, LVN and nurse aide hours in the same payroll data, Sharon Care Center was below 3.5 on 16 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

Sharon Care Center lawsuits and criminal cases

We searched California Attorney General and Los Angeles County District Attorney announcements and news coverage for Sharon Care Center and its licensee and found no public criminal case or civil lawsuit we could tie to this facility as of October 30, 2026. Many civil cases settle quietly or are not covered by the press, so this is not a complete court search.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
