April 2025: Class A citation and F687 (severity G) after a resident with a foot wound had a below-the-knee amputation

On March 18, 2025, CDPH made an unannounced visit to investigate a complaint about a resident's wound care. According to the citation, Resident 1 was a 70-year-old man with epilepsy, chronic kidney disease, heart failure and type 2 diabetes who depended on staff for daily care. A wound on the bottom of his left foot reopened in February 2025.

CDPH determined the facility failed to provide foot care that met professional standards. According to the citation, a wound physician specialist recommended an MRI on February 12, 2025, to rule out osteomyelitis (a bone infection), and the facility did not follow through. Inspectors found wound care was not documented on several days in February 2025, and no care plan was written for his left foot wound when he was readmitted in January 2024.

According to the citation, on February 19, 2025, the wound specialist suspected bone infection and possible wet gangrene. That day Resident 1 was sent to a hospital with sepsis (a life-threatening response to infection) from a necrotizing soft tissue infection (a fast-spreading infection that destroys tissue) of the left leg. Surgeons removed the foot at the ankle on February 20 and amputated the leg below the knee on February 27, 2025. CDPH issued a Class A citation with a $25,000 penalty, and the federal survey cited F687 at severity G, meaning actual harm.

Cal Health Find lists 127 complaint and facility-reported incident intakes for this facility from 2023 through October 30, 2026. CDPH substantiated at least one allegation in 42 of them. 47 intakes involved an allegation in CDPH’s abuse category, and 7 of those had an abuse allegation substantiated. An intake can contain several allegations; “unsubstantiated” means CDPH did not find enough evidence, not that nothing happened.

Sexual abuse reports at Kennedy Care Center

Cal Health Find lists 7 intakes for Kennedy Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (6 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 1 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
