July 2026: Class A citation and F689 (severity G) after a resident using a walker fell and broke a thigh bone
CDPH made an unannounced visit on June 10, 2026, to investigate a facility-reported fall. According to the citation, Resident 1 had ataxia (loss of muscle control), difficulty walking and repeated falls. A March 2026 assessment showed the resident needed supervision or touching assistance to walk, and an April 2026 fall risk assessment rated the resident at high risk for falls.
CDPH found the facility did not give Resident 1 that supervision or hands-on help, and did not develop a care plan that addressed the resident's fall risk. According to the citation, on May 21, 2026, Resident 1 walked to the activity room with a four-wheeled walker without assistance or supervision. The walker caught on the metal floor transition strip at the entrance and Resident 1 fell. The activity director told inspectors that at the time the director was helping another resident and was the only staff member in the room. A licensed vocational nurse told inspectors the resident should have been supervised and assisted by a certified nursing assistant.
According to the citation, Resident 1 broke the lower end of the left femur (thigh bone). The resident was taken to an acute care hospital, moved to a second hospital for trauma care, and had open reduction internal fixation (surgery to repair the bone with hardware) on May 26, 2026. When inspectors visited, they saw Resident 1 grimacing and holding the left knee. CDPH issued a Class A citation with a $25,000 penalty, and the 2567 report states the related federal survey cited F689 at severity G.
August 2024: F689 (severity G) after repeated falls by a resident with dementia
Inspectors found during an August 2024 complaint survey that the facility did not prevent repeated falls by Resident 1, who had Alzheimer's disease, dementia and a history of falls. According to the 2567 report, fall assessments in April and July 2024 rated Resident 1 at high risk for falls, and a July 2024 assessment found severe cognitive impairment.
The 2567 report states the facility did not develop and carry out a fall care plan after Resident 1 fell on June 30, July 24 and August 2, 2024, as its policy required. Inspectors found the care plans for impaired cognition and high fall risk did not include interventions specific to the resident, and did not say how staff would monitor or supervise the resident to prevent falls. Inspectors also found CNA 1 left Resident 1 sitting in a wheelchair in the hallway without staff supervision on August 2, 2024.
According to the report, on July 24, 2024, Resident 1 slid from the wheelchair and a CNA lowered the resident to the floor. A hospital CT scan showed a broken right hip, and the resident had a hip hemiarthroplasty (partial hip replacement) and returned to the facility on July 29. The report states that on August 2, 2024, the resident slipped from the wheelchair again, said repeatedly "I am hurt," and was taken by 911 to an acute care hospital. The new hip implant had dislocated and was put back in place without surgery. Inspectors cited F689, the rule on accident hazards and supervision, at severity G.

Hollenbeck Palms lawsuits and criminal cases
We searched California Attorney General and Los Angeles County District Attorney announcements and news coverage for Hollenbeck Palms and its licensee and found no public criminal case or civil lawsuit we could tie to this facility as of October 30, 2026. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
