April 2025: Class A citation and F689 (severity G) after a resident fell again and fractured his hip
On February 18, 2025, CDPH made an unannounced visit to investigate a facility-reported fall and to conduct an annual recertification survey. According to the citation, Resident 165 was a 92-year-old man with dementia, peripheral vascular disease (narrowed blood flow to the arms and legs), osteoarthritis and a history of falls. He used a walker, lacked the capacity to understand and make decisions, and scored 22 on a January 2025 fall risk assessment, which meant high risk.
CDPH found the facility did not revise his fall care plan with new interventions after he fell on the smoking patio on December 28, 2024. According to the citation, the interventions in his fall care plan, such as frequent visual checks and safety instructions, had been in place since October 2019, and his care plan for fracture risk had not been updated since February 2022.
According to the citation, on February 3, 2025, Resident 165 was again found sitting on the smoking patio floor, with pain of 7 out of 10 in his buttocks and both thighs. An x-ray on February 4 showed a left hip fracture, and he was taken to a hospital on February 5. The 2567 report states he had open reduction and internal fixation surgery (ORIF, a repair that holds the bone together with screws, plates or rods) on February 16, 2025. CDPH issued a Class A citation with a $25,000 penalty, and the federal survey cited F689 at severity G, meaning actual harm.

Cal Health Find lists 2 intakes for Burlington Convalescent Hospital from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (0 from complaints and 2 reported by the facility itself). |---|---|---|
| 2023 | 1 | 0 |

If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
