October 2025: Class A citation and F686 (severity J) after a bedbound resident developed a pressure injury
According to the citation, Resident 1, a 78-year-old man, returned to the facility on June 30, 2025, after a hip fracture, bedbound and dependent on staff to change position. His care plan called for skin checks every shift, repositioning every two hours and a pressure-relieving mattress or cushion. CDPH found his skin-risk assessment on return wrongly scored him as having no risk, and the director of nursing told inspectors there was no record those care plan steps were carried out.
On August 4, 2025, a wound specialist found a deep tissue injury (a type of pressure injury, or bed sore) over his tailbone with an open area and drainage, and recommended a low air loss mattress. The citation notes the resident declined the mattress that day; a family member who held his health care power of attorney told inspectors she was not told about the wound. CDPH found a high white blood cell count on August 7 was not reported to the wound specialist. On August 11 he was found unresponsive and was admitted to a hospital for sepsis (a severe reaction to infection). According to the citation, he died on August 14, 2025.
CDPH issued a Class A citation and assessed a $25,000 penalty. State records show the citation was appealed. The 2567 report cited F686, care to prevent and heal pressure ulcers, at severity J.
May 2023: F600 (severity G) and F609 (severity D) after a resident said her roommate pushed her
Inspectors investigating a complaint and two facility-reported incidents found that on April 29, 2023, Resident 3 fell to the floor, hit her head and bled from the mouth. According to the 2567 report, Resident 3 had intact cognition and told inspectors her roommate, Resident 4, pushed her. Resident 4 told inspectors she pushed Resident 3 to the ground.
The report states Resident 3 told a licensed vocational nurse (LVN 3) she had been pushed, and was told to get up and wash her face. LVN 3 told inspectors she saw Resident 3 throw herself down while she was in the supply closet; inspectors noted the closet was not directly across from the room. Another nurse (LVN 4) said Resident 3 also reported the push to him, that he considered it unsubstantiated, and that he did not follow the abuse policy. Inspectors found the two residents stayed in the same room after the incident. Resident 3 told inspectors she was terrified she could die if she was pushed again.
Inspectors cited F600, the right to be free from abuse and neglect, at severity G, finding the facility did not protect Resident 3, investigate her report or keep the two residents apart. Inspectors also cited F609, reporting of alleged violations, at severity D, finding the facility did not report the allegation as its policy requires.

Cal Health Find lists 1 intake for Alvarado Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (0 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
