June 2024: Class A citation and F689 (severity G) after a resident fell from a raised bed and fractured her hip

According to the citation, CDPH received a facility-reported incident on April 15, 2024 and made an unannounced visit on April 29. Resident 1, a 78-year-old woman with severely impaired thinking, was at high risk for falls and fully dependent on staff for daily care. Her care plan called for floor mats at her bedside and her bed kept in the lowest position.

CDPH found that on the night of April 11, 2024, CNA 2 found the resident soiled in bed and stepped away to get supplies from the bathroom, then heard a loud thud and found her face down on the floor. According to the citation, CNA 2 said the bed was at about bedside-table height, 30 to 36 inches, and CNA 1 saw no floor mat. The 2567 report states the facility did not have more than one staff member present for her incontinence care. The director of nursing told inspectors the bed should have been in its lowest position, with floor mats in place and two staff providing care.

According to the citation, the resident had a bump on her forehead, a scrape on her left arm and a non-displaced left intertrochanteric fracture (a break near the top of the thigh bone at the hip, with the bone still in line). She was sent to a hospital on April 12. CDPH issued a Class A citation for patient care with a $25,000 penalty, and the 2567 report cited F689 (severity G).

August 2023: F755 (severity K) after residents' ordered medications were not in the facility but were charted as given

According to the 2567 report, inspectors found the facility did not make sure four of five sampled residents received medications their physicians had ordered. The missed medications were a medication for behavior management for Resident 1, prednisone (an anti-inflammatory steroid) for knee inflammation for Resident 2, and two diabetes medications, Jardiance for Resident 3 and Trulicity for Resident 4.

Inspectors found licensed nurses documented these medications as given when they were not in the facility. The report states nurses did not confirm the medications had been received or follow up with the pharmacy, and the facility had no system to check whether the pharmacy received orders and delivered the drugs. According to the report, this put Resident 1 at risk of uncontrolled behavior, Resident 2 at risk of continued knee pain and loss of function, and Residents 3 and 4 at risk of high blood sugar, coma and possible death.

Inspectors called an immediate jeopardy on August 18, 2023 and removed it on August 21 after verifying the facility's removal plan, according to the report. The plan included notifying physicians and families, nurse assessments that found no adverse effects for Residents 2 and 4, two medication cart audits that found 19 and then four missing medications, a review of all 70 residents' medication records, and training for nurses on accurate documentation. The deficiency was cited as F755 (severity K), which covers pharmacy services.

Sexual abuse reports at Edgewater Skilled Nursing Center

Cal Health Find lists 2 intakes for Edgewater Skilled Nursing Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 1 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
