·
March 2026: Class A citation after a resident with a known history of sexual behavior entered another resident's room

CDPH made an unannounced visit on January 6, 2026, to investigate an allegation that Resident 2 sexually abused Resident 1. According to the citation, Resident 2, a 68-year-old man, was admitted on December 12, 2025, with a history of exposing himself and harassing female staff and residents. CDPH found the facility did not develop a care plan for this behavior at admission, or after he made sexually explicit comments to a nursing assistant on December 13 and touched a nursing assistant inappropriately on December 15.

According to the citation, Resident 1 was a 75-year-old woman with paralysis on one side after a stroke and dementia, and her physician's history said she did not have the capacity to consent. On December 19, 2025, Resident 2 entered her room while she rested in bed and lowered his pants. Resident 1 told inspectors he removed his pants and hers and kissed her face, and that she was shaken, slept with the lights on for two weeks and was afraid he would come back.

CDPH found the facility also left Resident 2 unsupervised with another resident, Resident 6, after he showed sexually inappropriate and aggressive behavior on December 19, and the citation states he physically abused Resident 6 the same day. According to the citation, these failures could cause psychological harm and placed other residents at risk. CDPH issued a Class A citation with a $20,000 penalty.

March 2024: F600 (severity G), F607 (severity D) and F609 (severity D) after a resident who wandered was hit with a back scratcher

The 2567 report states that at about 6:30 p.m. on March 13, 2024, CNA 1 saw Resident 2, in his wheelchair, hitting Resident 1 with a wooden back scratcher while she was lying on Resident 2's bed. According to the report, Resident 1 had dementia and diabetes, could not make her own decisions, and had a history of wandering into other residents' rooms and moving their belongings. Her care plan called for a sitter to monitor her constantly.

Inspectors found the facility did not monitor Resident 1's wandering, did not follow the care plan requiring a sitter, and did not follow its own abuse policy. LVN 2 told inspectors Resident 1 had a sitter on the morning shift that day but none that night, and that the incident could have been avoided with a sitter. Resident 2's roommate, who is blind, told inspectors he heard Resident 1 telling Resident 2 to stop hitting her, and that nurses knew she kept going into their room but did nothing about it.

The 2567 report states Resident 1 had a hematoma (a deep bruise) under her right eye, and a hospital emergency room diagnosed bruising around the eye. Inspectors cited F600 at severity G for failing to keep a resident free from abuse. The same survey cited F607 at severity D, which covers abuse and neglect policies, and F609 at severity D, which covers reporting of alleged violations. The full narrative for those two tags is in the linked report.

February 2023: Class A citation after vital signs were not checked for a resident with COVID-19 who was found unresponsive and died

CDPH received a complaint about quality of care on December 1, 2022, and made an unannounced visit the next day. According to the citation, Resident 1 was a 62-year-old man with difficulty swallowing, diabetes and dementia who was diagnosed with COVID-19 on November 25, 2022, and lived in the facility's isolation area. A physician's order and his care plan called for staff to check his vital signs every four hours.

CDPH found the facility did not check his vital signs every four hours as ordered and did not assess him when a nursing assistant saw he was weak and ate only three bites of breakfast on November 28. CNA 2 told inspectors she could not report his condition because there was no licensed vocational nurse in the isolation unit, and that she sometimes worked there without an assigned licensed nurse because of staffing problems. According to the citation, LVN 1 was called in because the assigned nurse called off, clocked in at 10:30 a.m., received no report, and found Resident 1 unresponsive at about 10:50 a.m. She started CPR while another staff member called 911.

According to the citation, Resident 1 died at the facility at 11:50 a.m. on November 28, 2022, seven hours after his vital signs were last checked at 4:30 a.m. His death certificate listed cardiopulmonary arrest and a myocardial infarction (heart attack). CDPH issued a Class A citation with a $20,000 penalty.

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
