·
August 2025: F689 (severity G) after a resident was moved to bed without a mechanical lift

The 2567 report states inspectors found the facility failed to make sure CNA 1 and CNA 2 used an assistive device such as a Hoyer lift (a mechanical lift for moving people with limited mobility) when they moved Resident 1 from her wheelchair to her bed, as her care plan required. According to the report, Resident 1 had paralysis and weakness on both sides of her body, could not stand, and depended on staff for transfers.

According to the report, on July 16, 2025, after a doctor's appointment, CNA 1 and CNA 2 moved Resident 1 back to bed. A family member outside the room heard her scream, and she said her leg hurt badly. Inspectors found she received the pain medication tramadol 14 times from July 16 to July 31, compared with 4 times in the first 15 days of July. On July 31 the family member raised concern about ankle swelling and asked for an X-ray, which showed recent fractures of the inner and outer ankle bones.

The report states Resident 1 was sent to an acute care hospital on August 2, 2025, and was diagnosed with a right trimalleolar fracture (a break in all three bony parts of the ankle). She had surgery under general anesthesia to fix the bones with hardware. The federal survey cited F689 (severity G) and also F609 (severity D), a tag on reporting alleged violations.

September 2023: Class A citation and F580 (severity G) after a resident's decline was not reported to her physician

CDPH received a complaint on August 8, 2023, that Resident 1 was non-responsive when her family visited, and made an unannounced visit that day. According to the citation, Resident 1 was a 76-year-old woman with diabetes and seizures who depended on kidney dialysis. A physician's order called for checking her every shift for changes in her level of consciousness.

According to the citation, CNA 2 said Resident 1 came back exhausted from dialysis on August 5 and would not wake up for dinner, and that she told LVN 1. CNA 1 said that on August 6 the resident was restless and called her by a different name, and she was not sure she reported it. On August 7, LVN 2 tried to reach her physician three times that morning without success, the director of nursing said she did not respond to a sternal rub (a firm rub on the breastbone) at about 11 a.m., and 911 was called at about 1 p.m.

CDPH found licensed nurses did not assess her or notify her physician on August 5, 6 and 7 and did not ensure a timely hospital transfer, so she received delayed emergency care. According to the citation, the hospital diagnosed acute encephalopathy (sudden brain dysfunction) due to a stroke and placed a feeding tube through her nose. CDPH issued a Class A citation with a $25,000 penalty, and the federal 2567 report cited F580 (severity G).

Sexual abuse reports at Camino Healthcare

Cal Health Find lists no complaint or facility-reported incident for Camino Healthcare from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse. That covers only what was reported to the state; if you suspect sexual abuse, report it even if nothing appears here.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
