What inspectors found at Rinaldi Convalescent Hospital

February 2024: F600 (severity G) after one resident punched another
The 2567 report states inspectors found the facility failed to protect Resident 1's right to be free from physical abuse when, on January 21, 2024, CNA 1 saw Resident 2 punch the right side of Resident 1's face. According to the report, Resident 1 had heart failure and moderately impaired thinking.
According to the report, at about 7 a.m. that day Resident 1 was yelling and screaming at the nursing station near Resident 2's room when Resident 2 came out and punched him. A progress note recorded minor abrasions (scrapes) on Resident 1's right temple. Resident 2 later had pain in his right hand, and a physician ordered an X-ray. Resident 2 told inspectors on February 5, 2024, that he hit Resident 1 because Resident 1 disturbed his sleep, and that he did not understand why staff let Resident 1 yell at night or early in the morning.
Inspectors found that a licensed vocational nurse described the incident as resident-to-resident physical abuse, and the director of nursing said staff were not able to stop it. The report notes the facility's own abuse prevention policy calls for protecting residents from abuse by anyone, including other residents. Applying the reasonable person concept, the report states that a person subjected to physical abuse can have lasting pain or psychological effects such as embarrassment and humiliation. The federal survey cited F600 (severity G).
February 2024: F921 (severity K) after roof leaks during a rainstorm
The 2567 report states inspectors found the facility failed to provide a safe environment for all 95 residents, staff and visitors because it did not keep its roof free of cracks and holes or its ceilings free of water damage. According to the report, 13 water leaks broke out across the building during a rainstorm on February 5, 2024. Three residents were displaced, and the report states the other 92 were put at risk of electrical shocks, electrical fires and possible structural damage.
Inspectors reviewed a September 2022 roof inspection that listed lifting roof edges, cracked sealant, exposed nails, low areas where water pooled and a torn cap sheet. A maintenance request from December 31, 2022, reported a dripping hole in a ceiling. On February 5, 2024, inspectors saw water leaking from a ceiling lamp in the lobby.
According to the report, the state survey agency declared an immediate jeopardy on the night of February 5, 2024. A removal plan was not accepted on February 7, a later plan was accepted on February 9, and the immediate jeopardy was removed on February 10. The accepted plan included frequent rounds to track leaks, moving residents out of affected rooms, temporary roof repairs and an electrical check by contractors, regular roof inspections, and a full roof replacement by January 1, 2025. The federal survey cited F921 (severity K).

Cal Health Find lists 11 intakes for Rinaldi Convalescent Hospital from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (3 from complaints and 8 reported by the facility itself). What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
