July 2025: Class A citation and F600 (severity G) after a resident punched another resident in the eye
According to the citation, on May 15, 2025, Resident 2 punched Resident 1, an 83-year-old woman with dementia, in the right eye, causing bruising and swelling. Resident 1 received pain medication, had emotional distress, told staff she did not feel safe in the facility, and said she did not want to be alone. CDPH determined the facility failed to prevent the physical abuse.
The department found Resident 2 had been diagnosed with schizophrenia and a mood disorder by a psychiatric evaluation in April 2025 and was prescribed a mood stabilizer and an antipsychotic, with orders to monitor her mood swings and hallucinations every shift. Inspectors found the facility never developed a care plan for her schizophrenia, and her admission record and assessment did not list that diagnosis. The citation states her medication records showed erratic mood swings on eight days and auditory hallucinations (hearing things that are not there) on five days between May 1 and May 13, 2025, with no description of the behavior or of what staff did in response.
According to the citation, the facility also failed to follow its abuse prevention policy, which called for staff to understand behavior that may raise the risk of abuse, such as outbursts and yelling, and failed to carry out a care plan that directed staff to listen to Resident 2 and address her concerns. The department issued a Class A citation with a $25,000 penalty. The federal 2567 report cited F600, freedom from abuse and neglect, at severity G, meaning actual harm.
January 2025: Class AA citation and F580 (severity G) after a delay in notifying a physician when a resident's oxygen level dropped
According to the citation, Resident 2 was a 73-year-old man admitted in November 2024 with cellulitis (a skin infection) in both legs, a blood clot in his left calf, and poor circulation in his legs. His assessment showed severely impaired cognition and four venous ulcers (open sores from poor blood flow) on his feet. CDPH opened its investigation after a complaint about his transfer to a hospital on December 29, 2024.
The citation states that at 3:08 a.m. that day, LVN 1 found discoloration and swelling of his right big toe and bleeding from his right leg, and he appeared confused. His oxygen level was 84% on room air. Over the next hour staff raised his oxygen from 5 to 15 liters per minute, but his level stayed between 84% and 87%. At 4:07 a.m. he had labored breathing, a cold arm, and responded only to touch. CDPH found 911 was called at 4:09 a.m. and he left for the hospital at 4:40 a.m., 92 minutes after the change was first seen.
The department determined the facility failed to notify his physician promptly of this significant change and failed to follow its own change-of-condition policy, which caused about a one-hour delay in his transfer. At the hospital he was diagnosed with septic shock (a severe infection causing very low blood pressure), sudden kidney failure and cellulitis. The citation states he died about 31 hours later. CDPH issued a Class AA citation for patient care with a $120,000 penalty. The federal 2567 report cited F580, notifying the physician of changes, at severity G, meaning actual harm.
January 2025: F600 (severity G) after two residents were struck by other residents, one suffering a broken nose
The 2567 report states the facility failed to make sure two residents were free from physical abuse in January 2025. On January 18, Resident 1 struck Resident 7 in the face without provocation in a hallway at Station A. On January 7, Resident 5 slapped Resident 6 on the right side of the face near the vending machines because Resident 6 would not light Resident 5's cigarette.
According to the report, Resident 1 had dementia and severely impaired cognition, and physician orders called for staff to monitor him every shift for sudden shifts from a pleasant mood to extreme anger and striking out at staff or peers. On January 7, he had slapped the hat off another resident, and a care plan started that day directed staff to distract and redirect him. Dietary Aide 1 told inspectors he saw Resident 1 hold Resident 7 by the shoulder with one hand and punch him in the nose with the other fist, that Resident 7 was too weak to push him away, and that he could not reach them in time to separate them.
Inspectors found nurses recorded no pain or visible injury for Resident 7 that day, but an X-ray on January 19, 2025, showed an acute depressed fracture of the nasal bone (a broken nose pushed inward). Resident 7 had dementia and severely impaired cognition. The survey cited F600, freedom from abuse and neglect, at severity G, meaning actual harm. The full narrative is in the linked report.
May 2024: F602 (severity J) after six residents' debit cards and cash were found in a social services office drawer
The 2567 report states that on May 17, 2024, at 10 a.m., inspectors saw six debit cards belonging to six residents, along with cash, in a drawer in the office of the social services designee. Inspectors found the facility failed to safeguard these residents' property and protect them from potential financial abuse, and that the social services staff member should not have had access to residents' financial documents, debit cards or cash.
According to the report, the facility failed to list the cards and cash on the residents' clothing and possessions lists, failed to give residents and their responsible parties receipts showing how much money was withdrawn from their cards, failed to keep the cards and cash in a locked cabinet, and had no system to track which residents had debit cards and money. Two of the cards belonged to residents who no longer lived at the facility. The inspectors determined this placed the six residents at risk of unauthorized use of their money and placed all 93 residents at risk of financial abuse.
An immediate jeopardy was called that afternoon in the presence of the director of nursing and the administrator. The report states the facility's removal plan included suspending and then firing the social services staff member, moving the cards and $27 in cash to a lockbox in the business office, ordering a safe, auditing every resident's inventory list, and retraining staff that only the business office may handle residents' money. Inspectors removed the immediate jeopardy on May 19, 2024. The survey cited F602, freedom from misappropriation and exploitation, at severity J, the immediate jeopardy level.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
