April 2025: F806 (severity J) after a resident with a gluten allergy was served cream of wheat
Inspectors conducted a certification survey at the facility in April 2025. The 2567 report states that Resident 71's physician's order, care plan, allergy list and other records listed an allergy to gluten. On April 8, 2025, the resident was served cream of wheat, a hot cereal that contains gluten, for breakfast.
According to the report, the meal ticket did not show cream of rice as the resident's choice, the kitchen had no cream of rice in stock, and tray line staff and LVN 3, who checked the tray, did not know cream of wheat should not be served to this resident. Inspectors found the food service manager or supervisor did not check the tray before it was sent out, as the facility's tray identification policy required. The report states this could have caused a life-threatening condition such as anaphylactic shock (a severe allergic reaction that can close the airways), cardiac arrest or death.
The 2567 report states the state survey agency called an Immediate Jeopardy over the error. In its removal plan, the facility said the director of nursing assessed Resident 71 on April 8 and found no adverse reaction, and that staff were trained on daily allergy reports, nurse checks of every tray against meal tickets and diet orders, and labeling gluten-free food. The survey cited F806 (resident allergies, preferences and substitutes) at severity J, and also F609 and F610 (reporting and investigating alleged violations), both at severity D.
April 2024: F806 (severity J) after a resident with a nut allergy was given a peanut butter sandwich
During a certification survey that began in April 2024, inspectors reviewed an incident from April 14, 2024. The 2567 report states Resident 147 was admitted on April 8, 2024, and the admission record, allergy report and nutritional assessment all listed an allergy to nuts.
According to the report, on April 14 Resident 147 asked for a snack. CNA 2 did not tell a licensed nurse, who would have had to complete a diet order and communication form before any food was given. Dietary Staff 1 gave CNA 2 a peanut butter and jelly sandwich without first getting that form, and CNA 2 gave it to the resident, who had asked for a tuna sandwich, according to a change of condition form. After a bite, Resident 147 complained of difficulty breathing, was placed on oxygen, was given Benadryl right away and was transferred to an acute care hospital.
The 2567 report states the state survey agency called an Immediate Jeopardy on May 1, 2024, and removed it on May 3 after verifying the facility's removal plan. The plan included one-on-one training for CNA 2 and Dietary Staff 1, interviews with all residents or their representatives to confirm food allergies, and a process requiring a licensed nurse to check the allergy report and complete the diet order form for any food request. The survey cited F806 (resident allergies, preferences and substitutes) at severity J.

CDPH substantiated at least one allegation in 26 of them. 51 intakes involved an allegation in CDPH’s abuse category, and 3 of those had an abuse allegation substantiated. Sexual abuse reports at Woodland Care Center
Cal Health Find lists 8 intakes for Woodland Care Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (1 from complaints and 7 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.
| Year | Intakes with a sexual abuse allegation | Substantiated |
|---|---|---|
| 2024 | 3 | 0 |
| 2025 | 1 | 0 |
| 2026 | 4 | 0 |

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
