November 2023: Class A citation, F684 (severity J) and F610 (severity D) after a resident's heel wound worsened and led to an amputation
According to the citation, CDPH made an unannounced visit on August 29, 2023, to investigate an allegation of resident neglect. Resident 1 had type 2 diabetes and chronic kidney disease, was fed through a stomach tube, and had a diabetic ulcer (a slow-healing wound) on the left heel. CDPH found the care plan called for blood sugar monitoring to help the wound heal, but there was no active order for the checks.
CDPH found the wound grew from 4.2 by 4.7 centimeters on August 8 to 4.4 by 5 centimeters on August 15, 2023, and the wound care nurse practitioner recommended a vascular consult (a specialist in arteries and veins). According to the citation, staff did not get a physician's order for that consult, did not hold an interdisciplinary care conference, and did not document weekly wound progress from August 2 to 21. CDPH also found the tube-feeding formula Resident 1 received did not match the physician's order.
According to the citation, Resident 1 was sent to an acute care hospital on August 23, 2023 and needed IV antibiotics and a left below-the-knee amputation. The hospital reported finding maggots (fly larvae) in the heel wound in the emergency department. CDPH issued a Class A citation with a $20,000 penalty. The 2567 report states inspectors called an immediate jeopardy on September 12, 2023, and lifted it on September 15 after the facility carried out a removal plan that included staff training. The federal survey cited F684 (quality of care) at severity J and F610 at severity D.

CDPH substantiated at least one allegation in 12 of them. 27 intakes involved an allegation in CDPH’s abuse category, and 3 of those had an abuse allegation substantiated. Sexual abuse reports at Culver West Health Center
Cal Health Find lists 4 intakes for Culver West Health Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (2 from complaints and 2 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 0 of them. | Year | Intakes with a sexual abuse allegation | Substantiated |
|---|---|---|
| 2024 | 3 | 0 |
| 2026 | 1 | 0 |
Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
