July 2026: Class A citation after a resident's sexual behavior toward other residents was not prevented

According to the citation, CDPH made an unannounced visit on May 20, 2026 to investigate a facility-reported incident of resident-to-resident sexual abuse. Resident 1 was admitted on March 28, 2026. His care plan noted a history, from hospital records, of inappropriate behavior toward female staff and attempts to touch residents, and it called for close supervision and hourly monitoring.

The department determined the facility did not monitor Resident 1 for aggressive behavior and verbal threats after he threatened to harm his new roommate, Resident 3, in April 2026. CDPH found staff did not keep close supervision of him, did not monitor his pacing and wandering at night, did not protect Resident 3 or report the threats to the administrator as facility policy required, did not document nurse monitoring after his change in condition, and did not keep an hourly monitoring log.

According to the citation, Resident 1 exposed and touched his genitals while staring at Resident 3, who was bedbound, and put Resident 3's toes in his mouth. Resident 3 told CDPH he felt disgusted and helpless because he could not move his body. CDPH found Resident 1 also went into Resident 4's room on the morning of May 19, 2026 and touched himself while looking at him, which made Resident 4 feel unsafe and afraid. Resident 2, also bedbound, said Resident 1 stood at her door and stared at her multiple times, which made her uncomfortable. CDPH issued a Class A citation in the Patient Rights category with a $25,000 penalty.

May 2026: Class A citation and F678 (severity J) after CPR was delayed for an unresponsive resident

According to the citation, CDPH made an unannounced visit on March 6, 2026 to investigate a complaint about quality of care. Resident 1 was admitted on February 26, 2026 with diabetes, congestive heart failure, dementia and atrial fibrillation (an irregular heartbeat). Resident 1's POLST (a form recording treatment wishes) said CPR should be attempted, so Resident 1 was a full code.

CDPH found that at 6:45 PM on March 5, 2026, Resident 1 was found weak, with shallow breathing, no longer talking and then unresponsive, with an oxygen level of 89%. The department determined the registered nurse supervisor (RNS 1) and LVN 1 did not start CPR right away. The citation states RNS 1 sat Resident 1 up at a 70 to 90 degree angle instead of laying Resident 1 flat on a firm surface and opening the airway, and staff did not keep CPR going until paramedics took over. CDPH also found CPR was delayed because no oxygen regulator was available that could deliver the 15 liters per minute needed for the bag-valve-mask (a hand-squeezed breathing device). When paramedics arrived at 6:52 PM, Resident 1 had no measurable blood pressure, pulse or breathing and no CPR was being done. Resident 1 was pronounced dead at 7:29 PM after 34 minutes of CPR by paramedics.

CDPH issued a Class A citation with a $60,000 penalty. The federal 2567 report cites F678 (CPR) at severity J. According to the 2567 report, the facility's removal plan included replacing oxygen regulators, checking the crash cart every shift, retraining staff on CPR, verifying CPR certifications and weekly Code Blue drills for three months.

October 2024: F689 (severity K) over unsupervised smoking by residents assessed as unsafe smokers

According to the 2567 report, inspectors found the facility did not follow its own resident smoking policy for eight residents who smoked. Seven of them had been assessed by the facility as unsafe smokers, and one had no completed smoking assessment. Inspectors found staff did not supervise them while they smoked and did not schedule a staff member to watch the smoking patio.

The report states Resident 3 kept cigarettes and lighters in a drawer, and Residents 56 and 67 were also allowed to keep cigarettes and lighters after being identified as not following the policy. Resident 14, assessed as unable to light tobacco safely, shared cigarettes with other residents and used another resident's lighter. Inspectors found a receptionist gave lit cigarettes to residents and let two of them smoke unsupervised outside scheduled smoking times. The 2567 report states these practices put the residents at risk of burns and fire that could cause serious injury, hospitalization and death.

The deficiency was cited under F689 (accident hazards, supervision and devices) at severity K. According to the report, CDPH identified an Immediate Jeopardy on October 22, 2024 and removed it on October 24 after verifying the facility's removal plan. The plan included taking residents' cigarettes and lighters into locked storage, assigning smoking attendants to watch smokers 24 hours a day, updating care plans, retraining the receptionist and educating residents who smoke on the policy.

September 2024: F689 (severity J) after a resident with severe cognitive impairment went missing

According to the 2567 report, Resident 1 had severely impaired cognition and dysphagia (difficulty swallowing). Inspectors found staff had seen her wandering into other residents' rooms and the front lobby, but the facility did not assess her as at risk for wandering and elopement (leaving without anyone knowing), did not develop a care plan with monitoring and supervision, and did not provide adequate supervision.

The report states Resident 1 was found missing around 1:15 PM on September 16, 2024. Inspectors found the facility did not call a Code Green (the overhead alert for a missing resident) right away and did not conduct a thorough investigation of how she left. According to the report, she missed her daily medications, was exposed to risks including extreme weather, malnutrition and death, and was still missing on September 19, 2024. The facility's own review indicated she likely left through the main lobby doors.

The deficiency was cited under F689 (accident hazards, supervision and devices) at severity J. CDPH visited on September 17, 2024, identified an Immediate Jeopardy that evening, and removed it on September 19 after verifying the facility's removal plan. The report states the plan included daily searches and calls to hospitals and police for three months, reassessing residents at risk, fixing sliding doors that opened wide enough to walk through, a 24-hour receptionist at the front doors, nurse checks every two hours, and staff training on elopement and Code Green.

Sexual abuse reports at Griffith Park Healthcare Center

Cal Health Find lists 11 intakes for Griffith Park Healthcare Center from 2023 through October 30, 2026 that included an allegation CDPH classified as sexual abuse (4 from complaints and 7 reported by the facility itself). CDPH substantiated a sexual abuse allegation in 4 of them. Federal rules require a nursing home to report an abuse allegation to the state within two hours, so a facility-reported incident means staff raised the allegation, not that it was proven. “Unsubstantiated” means CDPH did not find enough evidence; it does not mean nothing happened.

Warning signs include bruising or bleeding in the genital area or inner thighs, a new genital infection, torn or stained underclothing, and a resident who suddenly fears a particular staff member or resists being bathed or changed. If you suspect sexual abuse, call 911 or local police first, ask that the resident not be bathed before a medical exam, and then report it to CDPH and the long-term care ombudsman. The California average CMS published for the same period is 4.54, and the Los Angeles County median is 4.32. RN time averaged 0.38 hours per resident per day (California average 0.68). Counting the RN, LVN and nurse aide hours in the same payroll data, Griffith Park Healthcare Center was below 3.5 on 19 of 91 days. This is our calculation from CMS data, not a CDPH finding: CDPH counts some staff differently (for example psychiatric technicians in special treatment programs) and can grant waivers.

Griffith Park Healthcare Center lawsuits and criminal cases

What this record means for your family

California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.

Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.

Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
