November 2023: Class A citation and F689 (severity G) after a resident on non-weight-bearing precautions fell in a restroom and fractured a shoulder bone
According to the citation, CDPH found the violation during an unannounced annual recertification survey on October 10, 2023. Resident 19 had a history of stroke with right-sided weakness and had recently had surgery on a fractured right thigh bone near a previous knee surgery. Physician orders said Resident 19 must not put any weight on the right leg (non-weight-bearing) for six weeks. The facility had assessed Resident 19 as high risk for falls, and therapists found the resident could not do sit-to-stand or toilet transfers.
CDPH found the facility did not develop and carry out a care plan for two-person help with standing, toilet transfers and toilet hygiene, as physical and occupational therapy had recommended. According to the citation, on July 30, 2023, Resident 19 had help from only one person moving from the wheelchair to the toilet and standing up, and then had no support when stepping toward and standing at the sink. Resident 19 fell from a standing position onto the restroom floor. The next day the resident reported right shoulder pain, and an X-ray showed a right humeral (shoulder bone) fracture. The resident was transferred to an acute care hospital on August 1 and had shoulder surgery on August 3.
The department determined this was a Class A violation and assessed a $20,000 penalty. The federal 2567 report from the same survey cited F689, the rule on accident hazards and supervision, at severity G, which means actual harm.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
