August 2024: F689 (severity G) after a resident fell from a shower chair during a one-person mechanical lift transfer
According to the federal 2567 report, Resident 1 had dementia and one-sided weakness after a stroke, and depended completely on staff for transfers. The director of rehabilitation told inspectors Resident 1 always required a Hoyer lift (a mechanical lift) and two staff members for every transfer.
Inspectors found that on July 20, 2024, at about 11:30 a.m., CNA 1 used the Hoyer lift alone to move Resident 1 from the bed to a shower chair, contrary to the facility's lift policy. The report states Resident 1 fell from the shower chair to the floor when CNA 1 removed the lift strap from Resident 1's left shoulder, and a progress note recorded that Resident 1 hit the forehead on the bathroom doorknob. A registered nurse found Resident 1 on the floor bleeding from the head and called 911. At an acute care hospital, a 1-centimeter cut on the right forehead was closed with three stitches, and a CT scan showed a hematoma (a collection of blood) on the scalp.
According to the report, a facility corrective action memo dated July 25, 2024, stated CNA 1 did not follow directions for two-person assistance with the lift. The director of rehabilitation told inspectors the lift could tip over or the sling could break when one person operates it. Inspectors cited F689 (free of accident hazards, supervision and devices) at severity G.
February 2024: F689 (severity G) and F600 (severity D) after a resident was injured by a metal table during an unsupervised smoking session
According to the federal 2567 report, the facility failed to supervise a smoking session on its patio on February 19, 2024, involving Residents 1 and 2, who both had intact cognition. Inspectors found the two had a history of conflict: a February 15 room change form stated they yelled at each other, and a care plan told staff to step in and separate them during any altercation.
Resident 1 told inspectors that during smoking time, around 9 p.m., Resident 2 kept asking for a cigarette, became angry when refused and threw a patio table toward Resident 1. The report states Resident 2 pushed a metal table that struck Resident 1's right lower leg, and that a staff member, Receptionist 2, did not notice or intervene. Resident 1 said no staff were outside. A registered nurse told inspectors of finding Resident 1 in a wheelchair with blood dripping from the leg and seeing no staff on the patio.
According to the report, the cut on Resident 1's right lower leg was 14.3 centimeters long. Resident 1 was sent to an acute care hospital at about 9:25 p.m., needed 16 surgical staples to close it and reported pain of 7 out of 10. Inspectors cited F689 (free of accident hazards, supervision and devices) at severity G. The same survey also cited F600 (free from abuse and neglect) at severity D.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
