October 2024: Class AA citation and F684 (severity G) after a resident's breathing symptoms were not reported to a physician
CDPH investigated a facility-reported death in September 2024. According to the citation, Resident 1 was a 78-year-old woman with hydrocephalus (fluid buildup in the brain), diabetes and weakness on one side who received nutrition through a feeding tube and was at risk for aspiration (breathing food or fluid into the lungs). Her care plans told staff to report shortness of breath, wheezing and vomiting to the physician promptly.
Inspectors found that at about 7:45 a.m. on September 15, 2024, a CNA reported the resident was vomiting and sweating. LVN 2 found her short of breath and wheezing and gave a breathing treatment, anti-nausea medicine and Tylenol. She took one set of vital signs at 8:04 a.m. but recorded no follow-up vital signs or reassessment. LVN 2 told inspectors she did not notify the physician because she wanted to give medications to other residents, and that the resident's airway should have been the priority. RN 1 said LVN 2 did not report the symptoms. A CNA found the resident unresponsive at about 10:13 a.m., and paramedics pronounced her dead at 10:57 a.m. The assistant director of nursing told inspectors that if the physician had been notified earlier, the physician would probably have ordered an immediate X-ray to rule out aspiration.
CDPH issued a Class AA citation and assessed a $120,000 penalty, which the facility appealed. The federal 2567 report cited F684 (quality of care) at severity G.
May 2023: Class A citation and F684 (severity G) after a homemade warm compress burned a resident
CDPH received a complaint in March 2023 that a resident was burned by a hot pack prepared by staff. According to the citation, Resident 1 was a 75-year-old woman with diabetes and a spinal compression fracture who had impaired cognition. On March 4, 2023, a family member asked CNA 1 for a warm compress for her back pain.
Inspectors found CNA 1 made his own compress by wetting a towel with mixed hot and cold water from the dining room faucet, double-bagging it and putting it inside a pillowcase, then placed it on the resident's back under her draw sheet. He told inspectors he did not check its temperature, did not check on her afterward and did not tell the charge nurse. There was no physician's order for a warm compress. The facility stocked ready-made warm packs labeled at 110 degrees, and hot water from the dining room faucet measured 119 degrees. Facility policy calls for checking the skin every five minutes and says warm compresses should not be used on residents with diabetes.
According to the citation, the resident developed a fluid-filled blister measuring 10 by 10 centimeters on her back, and a wound doctor treated the open blister with Silvadene, an ointment used for burns. CDPH issued a Class A citation with a $20,000 penalty, and the federal 2567 report cited F684 (quality of care) at severity G.

What this record means for your family
California uses classes for state citations. Under Health and Safety Code section 1424 ↗, a Class AA citation is a Class A violation that CDPH determined was a direct proximate cause of a resident’s death. A Class A citation means CDPH found an imminent danger, or a substantial probability, that death or serious physical harm would result. Class B covers violations with a direct or immediate relationship to residents’ health, safety or security.
Federal deficiencies carry a letter from A to L. G, H and I mean inspectors found actual harm to residents; J, K and L mean immediate jeopardy, the most serious level. F600 is the federal rule that residents be free from abuse and neglect; F609 and F610 cover reporting and investigating allegations; F689 covers accidents and supervision; F686 covers pressure injuries.
Warning signs worth asking about: unexplained bruises or fractures, new bed sores, sudden weight loss or dehydration, a resident who becomes fearful or withdrawn around certain staff, and calls that go unanswered.
